摘要
目的了解广州市乙型肝炎病毒表面抗原(HBs Ag)阳性母亲所生新生儿乙肝免疫球蛋白(HBIG)及疫苗接种的应答水平及乙肝病毒变异情况。方法从广州市黄埔区、荔湾区、从化市、番禺区、天河区、越秀区6个区助产医院中,选取HBs Ag阳性产妇及其所分娩的接种乙肝免疫球蛋白及疫苗的新生儿,出生后接种HBIG及乙肝疫苗(3针),检测HBs Ag、抗乙型肝炎表面抗原抗体(抗-HBs)及乙肝病毒脱氧核糖核酸(HBV DNA)。结果共调查新生儿11 831名,HBs Ag筛查率为100.00%,其中HBs Ag阳性10名,抗-HBs阳性11 239名,HBs Ag携带率0.08%(10/11 831);对HBs Ag阴性但乙肝HBV DNA阳性者进行S基因测序,流行的乙肝病毒基因型有2种,其中B基因型70.00%、C基因型30.00%,不同毒株间核苷酸同源性为93.15%~99.91%;抗-HBs无(弱)应答者采取复种免疫后,抗-HBs阳转率达90.00%~92.00%,GMT(几何平均滴度)在260.02~304.37 m IU/ml间。结论广州市HBs Ag阳性母亲所生新生儿HBIG及疫苗接种效果明显;S基因序列和各参考株高度同源,未发现其他基因型及有生物学意义的变异;对无(弱)应答者采取复种免疫,抗-HBs阳转率显著提高。
Objective To understand the response level of Hepatitis B immunoglobulin (HBIG) and Hepatitis B virus (HBV) and Hepatitis B virus (HBV) variation in the Hepatitis B virus surface antigen (HBsAg) positive mothers in Guangzhou. Different HBV strains share the nucleotide homology of the S gene between 93.15% and 99.91%. Anti - HBs no (weak) respondents take revaccination after immunization, Anti - HBs positive rate of 90. 00% - 92.00% , GMT ( geometric mean titer) in 260. 02 - 304. 37 mIU/ml. Conclusion The effect of HBIG and vaccination of the newborn HBsAg in the mother of Guangzhou was obvious; The S gene sequence analysis of the prevalent HBV strains in Guangzhou showed the reference strains, and only two major HBV genotypes ( B and C) were found. For no (weak) respondents take revaccination immune, anti - HBs positive rate increased significantly.
