摘要
目的分析北京市2002年-2011年所收集的以根治术为主的食管癌患者住院费用的影响因素,找出影响结构变动的主要费用项目及其变动情况。方法运用新灰色关联分析法,对食管癌患者住院费用构成进行定量分析;采用结构变动度分析10年中食管癌患者住院费用结构的变动情况。结果 10年中食管癌药品费的关联系数和关联度最大,均为1.0000,其次为治疗费,再次为其他费用;10年来住院费用中其他费用结构变动贡献率最大,其次是治疗费,再次为检查费,三者的累计贡献率达到74.71%。结论药品费用的调控是控制食管癌患者住院费用的关键;含材料费的其他费用是食管癌患者住院费用变动的主要费用项目;食管癌患者住院费用的结构变动情况与疾病诊疗特点和治疗方式密切相关。建议控制医疗费用时应考虑疾病的治疗方式,也不要忽略材料费的控制。?
Objective To analyze the influencing factors of average hospitalization expense in patients with esophagus cancer in Beijing from 2002 to 2011,find out the main expense items that effected the cost structure as well as the variation situation.Methods New grey correlation analysis method was employed to quantitative analysis the hospital expenses proportion in patients with esophagus cancer.The change situation of 10 years hospital expenses structure was analyzed by degree of structure variation.Results In the 10 years,the correlation coefficient and relational degree of drug expense in patient with esophagus cancer was the greatest with 1.0000,followed by treatment expense and other expense.Other expense,treatment expense and inspection expense took the lead in contribution to structural changes of hospital expenses of esophagus cancer,which gave a cumulative contribution rate of 74.71%.Conclusions Drug expense in patients with esophagus cancer is the key for hospitalization expenses controlling.Other expense is the main changing item of hospitalization expenses in patients with esophagus cancer.The greatest item of the contribution rate of DSV（degree of structure variation） is closely related to their clinic features and treatment.It’s suggested that disease features and treatment should be considered and material expense cannot be ignored.
