摘要
目的:对比胸乳路径的腔镜甲状腺手术与开放性手术的临床效果。方法:笔者所在医院2013年1月-2016年1月共进行甲状腺良性肿瘤手术169例,按照手术方式分为腔镜组93例和开放组76例,腔镜组患者选择胸乳路径下腔镜甲状腺手术。开放组患者行开放手术。观察两组患者手术相关指标和对切口满意程度。结果:腔镜组患者的手术时间长于开放组,切口引流量多于开放组,术中出血量少于开放组,使用镇痛药物比例明显低于开放组,差异均有统计学意义(P<0.05)。两组患者的住院时间比较差异无统计学意义(P>0.05),两组均无不良反应发生。腔镜组患者对切口满意度明显高于开放组,差异有统计学意义(P<0.05)。结论:胸乳路径的腔镜甲状腺手术对患者损伤更小,美容效果更好。
Objective： To compare the clinical effect of breast approach endoscopic thyroid surgery and open surgery.Method： 169 cases who were admitted in our hospital for benign tumor of thyroid gland surgery during January 2013 to January 2016 were selected as the research objects and divided into laparoscopic group（93 cases） and open group（76 cases） according to the operation mode.The laparoscopic group was given breast approach endoscopic thyroid surgery.The open group underwent open surgery.Operation relevant indexes and the degree of satisfaction with the incision of two groups were observed.Result： The operation time of the laparoscopic group was longer than that of the open gronp, the wound drainage of the laparoscopic group was more than that of the open gronp, but the amount of bleeding during operation of the laparoscopic group was less than that of the open group and the use of analgesic drugs of the laparoscopic group was lower than that of the open group, the differences were statistically significant（P〈0.05）.The difference in the hospitalization time between the two groups was not statistically significant（P〉0.05）.There was no adverse reactions occurred in the two groups.The degree of satisfaction with the incision of the laparoscopic group was significantly higher than that of the open group, the difference was statistically significant（P〈0.05）.Conelusion： Breast approach endoscopic thyroid surgery on patients has less damage and better cosmetic results.
