摘要 目的 通过测定FENO在不同病因慢性咳嗽中的反应水平,以探究其在慢性咳嗽诊治中的临床意义.方法 本试验研究起始时间为2014年9月至2016年3月,研究对象为本院心内科诊断为慢性咳嗽的患者,试验的科学研究方法为前瞻性平行对照.根据中国《咳嗽的诊断及治疗指南》(2009版)中推荐的慢性咳嗽患者诊断方法,将所有纳入患者分为:变应性咳嗽(AC)、上气道咳嗽综合征(UACS)、咳嗽变异性哮喘(CVA);嗜酸粒细胞性支气管炎(EB)、胃食管反流性咳嗽(GERC)5个组别.在本院呼吸内科特设的FENO检测中心进行FENO测定.结果 检测结果显示,AC组(18.1±2.7)ppb、CVA组(38.2±1.4) ppb、UACS组(37.3±2.1) ppb、GERC组(13.6±1.0)ppb、EB组(19.2±4.1)ppb.FENO水平在不同组别比较显示,CVA组与UACS组差异无统计学意义(P>0.05),但二组均显著高于AC组、GERC组及EB组(P<0.05);各组别与GERC组比较差异均具有统计学意义(P<0.05),且其他组别的平均水平均增高.结论 FENO对慢性咳嗽患者的病因诊断和鉴别诊断及指导治疗具有一定的应用价值. Objective To explore the clinical significance of FENO in the diagnosis and treatment of chronic cough measuring FENO response levels in chronic cough caused by different etiologies.Methods This study started from September 2014 to March 2016.All the study objects were diagnosed with chronic cough at the department of Pediatric of our hospital.According to the diagnostic method of chronic cough recommended by "Chinese Cough Diagnosis and Treatment Guidelines"(2009 edition),the patients were divided into allergic cough,upper airway cough syndrome,cough variant asthma,addicted acid myelogenous bronchitis,and gastroesophageal reflux cough.The FENO was detected at the FENO center established by the department of respiration medicine at our hospital.Results The test results show that the FENO was (18.1±2.7) ppb in the AC group,(38.2±1.4) ppb in the CVA group,(37.3±2.1) ppb in UACS group ppb,(13.6±1.0) ppb in the GERC group,and (19.2±4.1) ppb in the EB group;there was no statistical difference between the CVA group and the UACS group (P 〉 0.05),but were between the above two groups and the AC group,the GERC group and the EB group (P 〈 0.05) and between the GERC group and the other groups (P 〈 0.05).Conclusions FENO has a certain value for the etiological and differential diagnosis and treatment of patients with chronic cough.
