摘要
马建教授将亚急性甲状腺炎的病机分为邪盛、正虚、情志失调三大方面,肝用失司是本病发病的重要病机。马建教授强调肝脾功能失调是亚急性甲状腺炎发病的基础,初起于肺脏,其主要病理因素包括气、热、痰、瘀等。由外感风热诱发,脾虚肝郁为发病基础。火毒,痰热,痰瘀凝结于颈前,气血流通不畅,则主要临床表现为疼痛。热毒之邪入血分,灼伤营阴,患者表现为夜间疼痛加剧,随之热势较盛。治疗以清热解毒、疏肝理气、消肿散结为原则:热毒炽盛者予清热解毒,散瘀止痛;痰热郁结者予清热化痰,软坚散结;脾虚痰湿者予健脾化湿,行气利水。
Professor Ma Jian thinks that pathogenesis of subacute thyroiditis can be divided into three aspects：Excessive pathogens, deficient healthy qi and emotional disorders, while the liver failing to control is an important pathogenesis of this disease. He emphasizes that liver dysfunction is the basic pathogenesis of subacute thyroiditis. It first appears in the lungs, and its main patho- logical factors include qi, heat, phlegm and blood stasis, etc. It is mainly induced by exogenous wind and heat and spleen deficiency and liver stagnation is its attack base. Fire-toxin, phlegm heat, phlegm stagnation coagulate in the neck,leading to poor qi and blood circulation clinically marked by pain. Heat-toxin enters xuefen, burns yin of yingqi, and makes the patient suffer from aggravating pain at night and the following excessive heat. The therapeutic principles are clearing heat and removing toxic substance, dispersing the stagnated liver-qi, reducing swelling and dispersing stagnation. For excessive heat-toxin, clearing heat and removing toxic substance, removing stagnation and stopping pain should be used ; For stagnated phlegm and heat, clearing heat and removing phlegm, and removing hard mass should be used;For spleen deficiency and phlegm dampness, activating the spleen and removing dampness, promoting qi circulation for inducing diuresis should be used.
