摘要
目的对比观察股骨近端防旋髓内钉(PFNA)与股骨近端髓内钉(PFN)治疗老年股骨粗隆间骨折的手术指标、血液相关指标及髋关节Harris评分。方法选取2015年2月至2016年2月收治的93例老年股骨粗隆间骨折患者,依手术方法不同设为对照组(41例)和研究组(52例),前者行PFN治疗,后者行PFNA治疗,对比观察两组手术指标、术后6 h、12 h、24 h血液相关指标及髋关节Harris评分。结果研究组手术时间、出血量、骨折愈合时间及负重时间均优于对照组(P<0.05);研究组术后不同时间超肌红蛋白(Myo)、超敏C反应蛋白(h-CRP)、肌酸磷酸酶(CK)水平均显著低于对照组(P<0.05);治疗后两组Harris评分均有提高,但研究组Harris评分明显高于对照组(P<0.05)。结论相比PFN,PFNA治疗股骨粗隆间骨折可改善手术指标与血液相关指标,提高Harris评分,值得临床推广应用。
Objective To analyze the efficacy, blood related indicators and Harris scores of PFNA and PFN fixation in treatment of elder- ly patients with femoral intertrochanteric fracture. Methods 93 elderly patients with femoral intertrochanteric fracture treated in this hospital dur- ing February 2015 to February 2016 were selected and they were divided into two groups： control group （41 cases） and study group （52 cases） according to the operational procedures. Patients in control group were treated with PFN internal fixation, and patients in study group were treated with PFNA internal fixation. The operational indicators and blood related indicators in 6h, 12h and 24h after the operation and Harris hip scores of patients in these two groups were obsserved and compared. Results The operating time, the amount of bleeding, fracture healing time and du- ration of weight bearing of patients in study group were better than those of patients in control group（ P 〈0.05）. Harris scores of these two groups were improved after treatment, but Harris scores in study group were evidently higher than those of control group （ P 〈 0.05）. Conclusion In comparison with PFN internal fixation, PFNA internal fixation in treatment of elder patients with intertrochanteric fracture can improve operational indicators and blood related indicators, and it can also raise Harris scores, hence it is worthy to be applied for clinical promotion.
