摘要
目的从中药中筛选耐甲氧西林金黄色葡萄球菌(methicillin resistant staphylococcus aureus,MRSA)外排系统的抑制剂。方法采用头孢西丁纸片法和PCR法鉴定供试菌株的耐药性及其外排基因norA与qacA/B;采用平板打孔法测定中药对MRSA41577的抑菌作用;采用联合药敏试验和双平板法测定中药对MRSA41577外排系统的抑制作用。结果 MRSA41577为耐药菌株,菌株内含有norA及qacA/B外排基因;24种中药粗提物中有22种对MRSA41577有抑制作用,只有水飞蓟和穿心莲对MRSA41577无抑制作用;联合药敏试验结果显示,水飞蓟和穿心莲分别与MRSA41577的耐药底物环丙沙星和苯扎氯铵联合作用后,均能恢复细菌对2种药物的敏感性,其中32 mg/m L的水飞蓟和穿心莲分别与环丙沙星和苯扎氯铵联用后,能使环丙沙星抑制MRSA41577的MIC由128μg/m L均降低至4μg/m L以下,苯扎氯铵抑制MRSA41577的MIC由4μg/m L分别降低至0.5μg/m L和1μg/m L;双平板结果显示,在含有终浓度为32μg/m L的环丙沙星和2μg/m L的苯扎氯铵平板中,加入水飞蓟和穿心莲后,能恢复环丙沙星和苯扎氯铵对MRSA41577的抑制作用。结论水飞蓟和穿心莲是MRSA41577的外排系统抑制剂。
Objective To screen MRSA efflux system inhibitors from traditional Chinese medicine. Methods Cefoxitin paper method and PCR method using resistance for the identification of strains and efflux gene of norA and qacA/B. Traditional Chinese medicine was evaluated by plate punching method for MRSA41577 bacteriostatic action. The inhibition of MRSA41577 efflux system was determined by combined chemosensitivity test and double-plate method. Results MRSA 41577 is a drug resistant strain,whith contained the efflux genes norA and qac A/B. 22 of 24 traditional chinese medicine showed inhibitory action to MRSA41577,only silymarin and Androgr Aphis paniculata have no inhibitory effect on MRSA41577. The results of combined chemosensitivity test showed that silymarin and Andrographis paniculata combinated with drug-resistant substrates of MRSA41577 Ciprofloxacin and benzalkonium chloride respectively,can restore the sensitivity of bacteria to the two drugs. Silymarin and andrographis paniculata at 32 mg/m L combined with ciprofloxacin and benzalkonium chloride respectively reduced the MIC of ciprofloxacin to MRSA41577 from 128 μg/m L to 4 μg/m L,the MIC of benzalkonium chloride decreased from 4 μg/m L to 0. 5 μg/m L and 1 μg/m L respectively. The results of the double-plate assay showed that,in the plate with ciprofloxacin at 32 μg/m L of final concentration and benzalkonium chloride at 2 μg/m L,the inhibitory effect of ciprofloxacin and benzalkonium chloride on MRSA41577 was restored when silymarin and andrographis paniculata were added. Conclusion Silymarin and andrographis paniculata are efflux system inhibitors of MRSA41577.
