超声引导下外侧膝眼进针膝关节腔注射与盲法膝关节腔注射的对照研究 被引量:15

COMPARISON OF SUCCESS RATE AND PUNCTURE PAIN INTENSITY BETWEEN ULTRASOUNDGUIDED AND PALPATION-GUIDED KNEE JOINT INTRAARTICULAR INJECTION

摘要 目的:观察超声引导下外侧膝眼进针膝关节腔注射与盲法膝关节腔注射成功率和疼痛程度的对比。方法:选取50名进行膝关节注射的患者,随机分为两组,A组,前两次行盲法膝关节腔注射,后两次行超声引导下膝关节腔注射;B组,前两次行超声引导下膝关节腔注射,后两次行盲法膝关节腔注射。每周治疗1次。操作后给予膝关节腔造影确定是否穿刺进入关节腔,并采用视觉模拟评分法(visual analogue scale,VAS)进行穿刺疼痛的评估。对比两组治疗的成功率和穿刺疼痛VAS。结果:超声引导下外侧膝眼进针膝关节腔注射的成功率为99.0%,盲法膝关节腔注射的成功率为82.0%,超声引导下外侧膝眼进针膝关节腔注射的成功率高于盲法膝关节腔注射,有统计学差异。两组之间穿刺疼痛VAS无明显统计学差异。结论:超声引导下外侧膝眼进针膝关节腔注射,成功率高,穿刺疼痛程度与盲法穿刺无明显差异,简易安全,是值得推荐的治疗方法。 Objective: To compare the success rate and pain intensity between ultrasound-guided and palpationguided knee joint intraarticular injection. Methods: Fifty patients treated with knee joint intraarticular injection were recruited. The treatments were implemented for four times consecutively with an interval of one week. The patients were divided randomly into two groups. In Group A, the first two treatments were implemented by palpation-guidance, the latter two by ultrasound-guidance. In Group B, the first two treatments were implemented by ultrasound-guidance, the latter two by palpation-guidance. Articular cavity radiography was implemented to judge the accuracy of the injection and pain intensity was also assessed by visual analogue scale(VAS) after the procedure. Results: The success rate of ultrasound-guided intraarticular injection was 99.0%, while that of palpation-guided injection was 82.0%. The former rate was significartly higher than the latter. There was no difference on puncture pain intensity between the two groups. No complications were found in both groups. Conclusion: Ultrasound-guided knee joint intraarticular injection is a convenient and safe procedure with high success rate.
