摘要
目的:探讨山东沿海地区人群血尿酸水平与血压水平的相关性。方法:2014年3~10月由青岛大学附属医院及当地卫生部门共同组成调查队采用随机分层整群抽样的方法对山东省沿海地区常住居民血尿酸水平与血压水平进行调查,并对该人群尿酸水平与血压水平的相关性进行分析。结果:共调查3157例(男1215例,女1942例),总人群高血压患病率为57。43%,其中男性患病率为64.52%,女性患病率为52.99%;总人群的高尿酸血症患病率为11.59%(366/3157),其中男性患病率为17.12%(208/1215),女性患病率为8.14%(158/1942),以上指标两组比较差异均有统计学意义(P〈O.001)。多因素有序多分类Logistic回归分析结果显示,尿酸水平在308~371μmol/L时,高血压前期患病风险最高(P〈0.01);而当尿酸水平≥257μmol/L时,高血压1级、2级和3级患病风险增加(P〈0.01),且随着尿酸水平的升高,高血压2级和3级的患病风险升高(P〈0.01),呈剂量反应关系。结论:人群中男性较女性高血压和高尿酸血症的患病风险高,且高血压患病风险随尿酸水平的升高而升高,血尿酸水平与高血压2级和3级的患病风险呈现剂量一反应关系,应加强该地区人群防治血尿酸升高的健康教育工作。
Objective: To investigate the correlation between the serum uric acid and blood pressure in the Shandong coastal areas. Methods: From March to October 2014, Random stratified cluster sampling was used to investigate the serum uric acid and blood pressure of residents in coastal areas of Shandong prov- ince. The correlation between uric acid level and blood pressure level in the population was analyzed. Re- sults: 3,157 cases (1,215 male and 1,942 female) were investigated. The prevalence of hypertension in male and female were respectively 64.52% and 52.99% with a total prevalence of 57. 43%. The prevalence of hyperurieemia was 17. 12% in male, 8.14% in female and 11.59% in total population. It showed significant differences between male and female in the above indexes (P 〈0. 01 ). Multiple logistic regression analysis showed that the prevalence of pre-hyperten-lence of grade Ⅰ to Ⅲ hypertension was increased when the level of uric acid was more than 257 μmol/L ( P 〈0.01). Moreover, the prevalence of grade Ⅰ and grade Ⅱ hypertension were increased concurrently with the elevated uric acid levels ( P〈0.01). Conclusion: The prevalence of hypertension and hyperuricemia in male were higher than those in female, and there was a positive correlation between serum uric acid and blood pressure. Health education work should be carried out in this area to prevent and control high uric r acid levels inthe blood.
