摘要
目的测定代谢综合征(MS)和2型糖尿病患者(T2DM)血清血管生成素样蛋白3(ANGPTL3)水平变化并探讨影响ANGPTL3的相关因素。方法选择2015年5月~2016年4月上海交通大学附属第六人民医院东院内分泌科150例住院患者。根据诊断分为代谢综合征(MS)组67例和非代谢综合征(NMS)组83例,及2型糖尿病(T2DM)组101例和对照组49例。采用液相因子浓度定量分析(Luminex)检测空腹血清ANGPTL3水平,测量腰围、臀围、收缩压、舒张压、血生化、血糖、糖化血红蛋白、胰岛素,计算体重指数(BMI)、腰臀比(WHR)及胰岛素抵抗指数(HOMA-IR)。结果MS组血清ANGPTL3水平[(3.40±1.60)ng/mL]高于NMS[(2.66±1.33)ng/mL],差异有高度统计学意义(P=0.003);T2DM组血清ANGPTL3水平[(3.17±1.46)ng/mL]高于对照组[(2.62±1.51)ng/mL],差异有统计学意义(P=0.037)。Spearman相关分析显示,血清ANGPTL3水平与腰围、WHR、BMI、HOMA-IR、空腹血糖、空腹胰岛素、三酰甘油呈正相关关系(P〈0.05)。进一步以血清ANGPTL3为因变量,以腰围、BMI、WHR、HOMA-IR、三酰甘油、空腹血糖、空腹胰岛素为自变量进行多元逐步回归分析显示,三酰甘油是血清ANGPTL3水平的独立相关因素(P〈0.01)。结论血清ANGPTL3水平在MS和T2DM人群中均升高,三酰甘油是ANGPTL3水平的独立相关因素,提示ANGPTL3分泌蛋白参与MS发病。
Objective To evaluate the level and relevant factors of serum ANGPTL3 in patients with metabolic syn- drome(MS) and type 2 diabetesmellitus (T2DM). Methods A total of 150 patients in the Department of Endocrinology, the East Hospital of Sixth People's Hospital, Shanghai Jiaotong University the were selected from May 2015 to April 2016. According diagnosis, these patients were divided into the MS group (n=67) and the NMS group (n=83),and the T2DM group (n=101) and the control group (n=49). The fasting serum concentration of ANGPTL3 was detected by liq- uidphase factor concentration quantitative analysis (Luminex). Waist circumference, hip circumference, systolic pres- sure, diastolic pressure, blood biochemistry, glucose levels, glycosylated hemoglobin (HbAlc) and insulin were measured. The body mass index(BMI), waist to hip ratio (WHR) and insulin resistanceindex (HOMA-IR) were calculated. Results Serum level of ANGFFL3 of the MS group [(3.40±1.60) ng/mL] was significantly higher than that of NMS group [(2.66± 1.33) ng/mL], with statistically significant difference (P=0.003). Serum ANGPTL3 levels of the T2DM group [(3.17±1.46) ng/mL] was higher than that of the control group [(2.62±1.51) ng/mL], with statistically significant difference (P = 0.037). Spearman correlation analysis indicated that serum ANGPTL3 was positively associated with waist circumference, WHR, BMI, HOMA-IR, fasting blood glucose, fasting insulin, triglycerides (P 〈 0.05). Furthermore, multiple linear step- wise regression analysis showed the triglyceride was an independent risk factor for serum ANGPTL3 level (P〈0.01) after adjusting for the effects of waist circumference, BMI, WHR, HOMA-IR, triglyeeride, fasting glucose and fasting insulin. Conclusion Serum level of ANGPTL3 is elevated in the patients with MS and T2DM, and is independently correlatedwith triglyceride, which indicates that the secretory pro- tein may be involved in the pathogenesis of MS.
