摘要
目的:通过对1例自身免疫性溶血性贫血患者(AIHA)Rh系统类抗体的鉴定与分析,指导临床安全、有效地用血。方法:应用盐水法、抗人球蛋白法及酸放散等试验对患者血浆和放散液中的抗体进行鉴定,并结合其自身红细胞血型抗原分析其抗体特异性。结果:患者血浆及放散液中抗体具有抗-D特异性,且自身红细胞抗原Rh D为阳性,具备Rh血型系统类抗体的血清学特征,经鉴定为类抗-D抗体。予D抗原阴性血液配合性输注,临床输注有效。结论:少数AIHA患者自身抗体具有类Rh血型系统特异性,在临床输血中应予相应抗原阴性的血液配合性输注,保障临床用血安全、有效。
Objective：To offer guide for the safety and effectiveness of clinical blood use by identification and analysis of Rh blood group system similar antibodies in patients with autoimmune hemolytic anemia （ AIHA）. Methods ： Saline method, antiglobulin test and acid elution test were employed to identify the antibodies in plasma and elution, and the antibodies＇specificity was analyzed by combining its own red blood cell group antigen. Results：The antibodies had the serological characteristics of Rh blood group system which were identified as similar anti-D antibody. The antibodies in plasma and elution had anti-D specificity and its red blood cell RhD antigen were positive. The clinical effect was better in the case of cooperative D antigen-negative blood transfusion. Conclusion ： The small number of AIHA patients with the specificity of similar Rh blood group system should be infused with corresponding antigen negative blood with antibodies in order to ensure the safety and effectiveness of clinical blood use.
