富血小板血浆复合自体髂骨治疗骨不连的临床疗效 被引量:18

摘要 目的探讨富血小板血浆(PRP)复合自体髂骨治疗骨不连的方法及疗效。方法2013年8月至2015年1月,采取随机对照研究方法,选取中国人民解放军第八十九医院创伤骨科收治骨不连患者56例,将骨不连患者以单盲法按数字随机分为PRP复合自体髂骨组(观察组)、单纯取髂骨植骨组(对照组),每组28例患者,对比两组患者术后1、2、3个月骨痂生长情况(根据上海骨伤科研究所制定的X线片骨痂评定标准)以及骨折最终愈合情况并行统计学分析,采用两组独立样本的t检验比较临床疗效。结果56例患者均获得随访,随访时间为10~28个月。术后1、2、3个月骨折断端X线骨痂评分:观察组明显高于对照组,组问差异有统计学意义(t1=2.88,P〈0.05;t2=13.04,P〈0.05;t,=20.56,P〈0.05),至随访结束,观察组患者的临床愈合时间、骨性愈合时间明显短于对照组,组间差异有统计学意义(t=8.197,P〈0.05;t=9.107,P〈0.05),观察组全部达到骨性愈合,而对照组组仅有24例患者最终获得骨性愈合,4例患者再次发生骨不连。结论PRP复合自体髂骨可加快骨折断端新骨的形成,提高新生骨的质量,是治疗骨不连的一种有效方法。 Objective To evaluate the effects of the platelet-rich plasma (PRP) combined with autogenous iliac graft in treating bone nonunion. Methods From Aug 1,2013 to Jan 1,2015, 56 patients of bone nonunion were treated in the Department of Orthopedics and Traumatology, the 89th Hospital of PLA. According to the randomized controlled study method, the patients were randomly divided into two groups : the PRP composite iliac crest group ( observation group) and the iliac bone grafting group ( control group) , 28 patients in each group. Two groups of patients were compared for the callus growth at the 1 ^st, 2^nd, and 3^rd month after the surgery (according to the X-ray callus evaluation criteria developed by Shanghai Orthopedic Trauma Institute); the final healing of the bone fractures was also parallel statistical analyzed. The independent t-test was used to compare the clinical efficacy of the two groups. Results Fifty-six cases were followed-up for 10 to 28 months. The bone callus score of X-ray at 1^st, 2^nd, and 3^rd months after the operation was significantly higher in the observation group than in the control group; significant difference was found between them ( t1 = 2. 88, P 〈 0.05 ; t2 = 13.04, P 〈 0. 05 ; t3 = 20. 56, P 〈 0. 05). At the end of the follow-up, the clinical healing time and the bone healing time were significantly shorter in the observation group than in the experimental group ( t = 8. 197, P 〈 0.05 ; t = 9. 107, P 〈 0. 05 ). In the control group, only 24 patients eventually got bone union, and four patients recurred nonunion. Conclusion Platelet-rich plasma autogenous iliac bone autograft can accelerate the new bone formation and improve the quality of new bone, which is an effective method for the treatment of bone nonunion.
