摘要
目的观察化浊通督针法治疗中风后假性球麻痹(PBP)的临床疗效,并探讨其对患者吞咽功能及构音功能的影响。方法将80例中风后PBP患者随机分为2组。对照组40例,予内科常规治疗;治疗组40例,在对照组的基础上加用化浊通督针法治疗。观察比较2组临床疗效,并比较2组治疗前后吞咽功能及构音功能的变化,吞咽功能采用洼田氏饮水试验评定,构音功能采用改良Frenchay构音障碍评估量表评定。结果治疗组治疗后总有效率90.0%,对照组总有效率77.5%,2组总有效率比较差异有统计学意义(P<0.05),治疗组疗效优于对照组;2组治疗后吞咽功能洼田氏饮水试验分级情况较本组治疗前均有改善,比较差异均有统计学意义(P<0.05),且治疗组治疗后吞咽功能洼田氏饮水试验分级情况较对照组治疗后改善更明显(P<0.05);2组治疗后构音功能改良Frenchay量表评分较治疗前均有提高,比较差异均有统计学意义(P<0.05),且治疗组治疗后构音功能改良Frenchay量表评分较对照组治疗后提高更明显(P<0.05)。结论化浊通督针法治疗中风后PBP临床疗效确切,可明显改善患者的临床症状,改善患者的吞咽功能及构音功能,促进神经功能恢复,提高患者生活质量。
Objective To observe the clinical effects of Huazhuo-tongdu acupuncture on the treatment of post-stroke pseudobulbar palsy（PBP） and its effects on swallowing and articulating function. Methods 80 patients with post-stroke PBP were randomly divided into two groups. 40 cases in control group were treated by routine western medicine treatment. 40 cases in treatment group were treated by Huazhuo-tongdu acupuncture on the basis of control group treatment. The clinical effect was compared between two groups. The swallowing and articulating function before and after treatment were compared between two groups. The swallowing function was evaluated by water swallow test,the articulating function was evaluated by modified Frenchay scale. Results The total effective rate in treatment and control group was 90. 0% and 77. 5%respectively,with statistical differences（P〈0.05）,the treatment group with better curative effects. The grade of water swallow test after treatment was improved in two groups,with statistical differences（P〈0.05）,and the improvement in treatment group was superior to that in control group（P〈0.05）. The scores of modified Frenchay scale after treatment was improved in two groups,with statistical differences（P〈0.05）,and the improvement in treatment group was superior to that in control group（P〈0.05）. Conclusion Huazhuo-tongdu acupuncture has exact effect on the treatment of post-stroke PBP,can obviously improve the clinical symptoms,swallowing and articulating function,can promote neural function recovery,enhance the quality of life.
