重组人类基因促红细胞生成素对早产儿脑损伤的保护作用及临床疗效 被引量:7

Protective and Clinical Effect of rhEPO in Cerebral Injury of Premature Infants

摘要 目的:探讨重组人类基因促红细胞生成素(rhEPO)对早产儿脑损伤保护作用及临床疗效。方法:选择2012年1月至2014年5月我院出生并收治的76例早产儿为研究对象。将76例患儿随机分为对照组与观察组各38例。两组患儿均在出生后给予常规治疗,观察组自出生起即在基础治疗上加用rhEPO。对所有病例严格随访至出生后1岁。结果:两组患儿出生时的新生儿神经行为测定(NBNA)评分分别为28.72±5.56与29.29±5.16,两组间比较未见显著性差异(P>0.05),经治疗12个月观察组NBNA评分为39.32±7.58,对照组为35.87±6.93,两组均较治疗前有了显著提高,组间比较观察组评分高于对照组,差异具有显著性(P<0.05)。对两组进行Gesell发育量表评价,观察组早产儿的精细动作、适应能力及社交行为得分均高于对照组,差异具有统计学意义(P<0.05)。观察组共11例早产儿表现为神经系统影像学异常,发生率为28.9%,对照组为21例,发生率为55.3%,组间比较观察组异常率低于对照组,差异具有显著性(P<0.05)。结论:rhEPO对神经组织具有直接的保护和营养作用,对早产儿使用rhEPO可有效改善其预后,降低脑损伤后遗症的发生,可作为临床治疗早产儿脑损伤的一种新方法加以推广应用。 Objective:To study the protective and clinical effect of rh-EPO in cerebral injury for premature infants.Methods:76premature infants treated from January 2012 to May 2014 in our hospital were selected and randomly divided into control group and rh-EPO group,with 38 cases in each group,and followed up for one year.Results:Upon their birth,NBNA scores of two groups were 28.72±5.56 and 29.29±5.16,respectively(P〉0.05);after 12 months treatment,NBNA score of rh-EPO group was 39.32±7.58 while the score of control group was 35.87±6.93;Significant difference was foud in NBNA after treatment and between the two groups(P〈0.05);according to Gesell development scale,the score of fine movement,adaptive capacity and social behavior of rh-EPO group was higher than that of control group(P〈0.05);the rh-EPO group had11 cases with abnormal imaging in the nervous system and its incidence rate was 28.9%,whilethe control group had 21 abnormal cases and its incidence rate was 55.3%.The abnormal rate of rh-EPO group was lower than that of control group(P〈0.05).Conclusion:EPO can play a direct role in the protection and nutrition supply for nervous tissue.For premature infants,the application of EPO can greatly improve the prognosis and reduce the incidence of complications after brain injury.EPO is useful in the clinical treatment of brain injury for premature infants.
