摘要
目的探讨超声心动图早期检出胎儿心律失常并评价其预后的临床价值。方法应用超声心动图检出患有心律失常的胎儿共56例,多种超声方法联合应用分析其心律失常的类型,并观察是否并发心脏结构异常及其类型。结果胎儿心律失常伴发心脏结构异常的比例为4/56(7.14%),且缓慢性心律失常伴发心脏结构异常的比例明显高于其他类型的心律失常。得到随访的52例心律失常胎儿中,除3例家属要求终止妊娠及1例经临床积极治疗转为窦性心律外,其余病例均逐渐转为正常心律。结论应用超声心动图多种方法联合诊断胎儿心律失常,评估胎儿心律失常的疗效及预测心律失常胎儿的预后均有重要价值。
Objective To explore the detection of fetal arrhythmia at early stage via ultrasonic cardio-gram and evaluate its prognostic clinical value. Methods Fifty-six cases of fetuses with arrhythmia weredetected by applying ultrasonic cardiogram. Multiple ultrasonic methods were applied in combination toanalyze the types of arrhythmia and observe if they were complicated with cardiac structural abnormality aswell as their types. Results The proportion of fetal arrhythmia accompanied by cardiac structural abnor-mality was 7.14% （4/56）, and the proportion of chronic arrhythmia accompanied by cardiac structuralabnormality was significantly higher than that of arrhythmia of other types. Among the follow-up 52 casesof fetal arrhythmia, 48 cases had all gradually transferred into normal rhythm, except 3 cases terminatedpregnancy at the request of family members and 1 case transferred into sinus rhythm through clinical activetreatment. Conclusion It is of great value to diagnose fetal arrhythmia, evaluate the efficacy of fetal ar-rhythmia and forecast the prognosis of fetuses with arrhythmia by utilizing multiple methods of ultrasoniccardiogram.
