摘要
目的 探讨Ⅰ期子宫内膜癌外科治疗后放射治疗的生存状况. 方法 选取2008年3月～2011年2月在中山市人民医院进行外科治疗后放射治疗的Ⅰ期子宫内膜癌患者65例,分析患者3年、5年总生存率、无瘤生存率、局部复发、远处转移率及不良反应总发生率. 结果 Ⅰ期子宫内膜癌外科治疗后放射治疗的3年总生存率、无瘤生存率分别为100％、98.26％;5年总生存率、无瘤生存率分别为90.38％、86.54％.局部复发3例（4.62％）,远处转移4例（6.15％）.急性放射性肠炎11例（16.92％）,急性放射性膀胱炎7例（10.77％）,肠穿孔1例（1.54％）,肠梗阻2例（3.08％）,便血4例（6.15％）.不同分化程度、年龄及病理类型与Ⅰ期子宫内膜癌外科治疗后放射治疗患者5年生存率具有一定相关性（RR=5.158,P=0.023）. 结论 以子宫内膜样腺癌为主的Ⅰ期子宫内膜癌患者进行术后辅助放疗的疗效良好,放疗后不良反应发生率低,不易发生局部复发及远处转移.
Objective To investigate the living conditions of postoperative radiotherapy for ents stage I endometrial carcinoma （ EC） . Methods 65 patients with stage I endometrial noma were treated with hysterectomy and postoperative radiotherapy in Zhongshan Peoples Hospital March, 2008 to Feburary, 2012 were included in this study. The 3-year overall survival, 5 -year all survival, disease-free survival, local recurrence and distant metastasis rates for all patients were yzed. Results The 3-year overall survival and disease-free survival rate in patients were 100%, 26% , respectively; 5-year overall survival, disease-free survival rate were 90. 38%, 86. 54%, ectively. Local recurrence in 3 cases （4. 62%）, distant metastasis 4 cases （6. 15%）. Acute ation enteritis 11 cases （16.92%） , acute radiation cystitis 7 cases （10. 77%） , bowel perforation in se （1.54%）, intestinal obstruction in 2 cases （3. 08%）, blood in the stool 4 cases （6. 15%） . The rences in the degree of differentiation, age and pathological types has certain correlation with the 5- overall survival rate in patients stage I endometrial carcinoma and received postoperative otherapy. Conclusions For patients with stage I EC （ mostly adenoeareinoma）, postoperative otherapy has good efficacy which could achieve a low incidence of adverse reactions after otherapy, less local recurrence and distant metastasis.
