乳腺超声图像报告与数据系统在乳腺良恶性病变诊断中的应用价值 被引量:7

摘要 目的评价超声乳腺影像报告数据系统(BI-RADS-US)分级在乳腺良恶性病变中的诊断价值。方法应用高频超声对在本院术前经超声检查发现的肿块并经病理证实的782例女性患者乳腺肿块进行回顾性分析,比较分析其超声BI-RADS分级评估与病理检查结果,计算超声诊断的乳腺良、恶性肿块的恶性肿瘤百分率,并比较不同年龄段乳腺恶性肿瘤的百分率。结果 782例患者中,超声诊断恶性肿块168例,其中113例病理诊断为恶性肿瘤,654例病理诊断为良性病变;超声诊断良性肿块614例,其中15例病理诊断为恶性肿瘤,599例病理诊断为良性病变。由超声BI-RADS对乳腺肿块进行诊断,灵敏度88.28%、特异度91.59%、总体符合率为91.04%、阳性预测值67.26%、阴性预测值97.56%。结论 BI-RADS分类使超声能够对乳腺良恶性病变进行较准确的评估,对规范诊断与临床处理有较高的参考价值,提升了超声报告质量,有利于与其他影像交流及临床医师对超声提示的理解。 Objective To discuss the value of breast imaging reporting and data system ultrasonography(BI-RADS-US) classifcation in the diagnosis of breast lesions. Methods In our hospital, 782 cases of female patients with breast lesions were retrospectively analyzed by preoperative ultrasound examination, parallel operation, biopsy pathology, and were compared their BI-RADS ultrasound gradings with pathological findings. Then the malignant rates of breast lesions with benign sonographie descriptor and breast lesions with malignant sonographie descriptor were calculated. And the malignant rates of different ages were compared. Results In 782 patients, ultrasonic diagnosis of malignant tumor in 168 cases. Among them, 113 cases were diagnosed as malignant tumors. 654 cases were diagnosed as benign lesions. Ultrasonic diagnosis of benign tumor in 614 cases, among them, 15 cases were diagnosed as malignant tumors, 599 cases were diagnosed as benign lesions. The diagnosis of breast masses by ultrasonic BI-RADS, sensitivity 88.28%, specificity 91.59%, the overall compliance rate was 91.04%, positive predictive value 67.26%, negative predictive value 97.56%. Conclusion BI-RADS grading to breast lesions has more accurate assessment, which are of a high reference value for the standardization of diagnosis and clinical treatment.
