ALA-PDT联合氩离子束凝固术治疗肛周巨大尖锐湿疣45例临床观察 被引量:10

Combination of 5-Aminolevulinic Acid-Photodynamic Therapy with Argon Plasma Coagulation in Treatment of Perianal Giant Condylomata Acuminata

摘要 目的评价5-氨基酮戊酸光动力疗法(5-aminolevulinic acid-photodynamic therapy,ALA-PDT)联合氩离子束凝固术(argon plasma coagulation,APC)治疗肛周巨大尖锐湿疣(giant condyloma acuminata,GCA)的临床疗效及安全性。方法将入选的90例肛周GCA患者采用简单随机方法分为治疗组及对照组,每组45例,全部患者予APC去除可见疣体,1周后治疗组患者(45例)采用ALA-PDT治疗,1次/周,连续3周。治疗结束后两组患者均随访6个月。结果随访结束时,治疗组临床痊愈41例,复发4例,复发率9.76%;对照组临床痊愈34例,复发11例,复发率32.35%,两组复发率比较:差异有统计学意义(P均<0.05),即治疗组的临床疗效明显优于对照组。结论 ALA-PDT联合APC治疗肛周GCA疗效确切,治愈率高,复发率低。 Objective To evaluate the efficacy and safety of 5-aminolevulinie acid-photodynamic therapy (ALA-PDT) combined with argoa plasma coagulation (APC) in treatment of giant perianal condylornata acuminata (GCA). Methods Ninety patients with perianal GCA were randomly and evenly divided into treatment group and control group. The visible lesions were removed by APC in both groups. ALA-PDT was introduced one week later in the treatment group for three times, and responses to treatment were assessed. Results At the end of six-month follow-up, forty-one cases from treatment group were shown to be cured, recurrence was found in four cases from the same group; however, there were thirty-four controls found to be cured, along with recurrence in eleven controls. The recurrence rate was 9.76% and 32. 35% respectively, and the difference was significant (P 〈 0. 05). Conclusion The combination therapy of ALA-PDT with APC is a safe and effective regimen in the treatment of perianal GCA because it demonstrated higher curative rate and lower recurrence than argon plasma coagulation alone.
