摘要
目的探讨锁定钛板非植骨手术在治疗Sanders 2、3型跟骨骨折中的应用效果。方法选择2014年4月~2015年6月我院接诊的50名采用锁定钛板非植骨手术治疗的Sanders 2、3型跟骨骨折的患者进行研究。按随机抽取的方式分为对照组和实验组各25例,实验组患者未行植骨复位后,行锁定钛板内固定医疗。对照组患者取自体骨植骨复位后,行锁定钛板内固定,随访12~18个月。观察两组患者手术前后bohler角和Gissane角的变化、手术指标及住院时间以及两组患者的术后疗效。结果术后两组患者的bohler角、Gissane角与术前比较,明显提高（P〈0.05）;实验组患者的bohler角和Gissane角明显高于对照组,差异具有统计学意义（P〈0.05）;实验组患者住院时间、手术时间、出血量均明显少于对照组,差异具有统计学意义（P〈0.01）;实验组患者的功能恢复优良率明显高于对照组,差异具有统计学意义（χ^2=10.860,P〈0.01）。结论锁定钛板非植骨手术在临床治疗Sanders 2、3型跟骨骨折中效果显著,患者术后恢复良好,值得临床广泛推广。
Objective To explore the application effect of locking titanium plate non bone graft surgery in treatment of calcaneal fractures with Sanders type 2 and type 3. Methods 50 cases of patients with calcaneal fractures of sanders type 2 and type 3 treated with locking titanium plate non bone graft surgery in our hospital grom April 2014 to June 2015 were selected as the study objects. According to random sampling method, they were divided into control group and experiment group with 25 case in each. Patients in experiment group were treated with locking titanium plate internal fixation, without bone graft, and patients in control group were treated with locking titanium plate internal fixation after bone graft, and follow up 12-18 months. Changes of Bohler angle and Gissane angle, operation index, length of stay and postoperative curative effect of two groups of patients before and after operation were observed. Results Compared with that before operation, the Bohler angle and Gissane angle of the two groups were significantly increased（P〈0.05）. The Bohler angle and Gissane angle of experiment group were significantly higher than those of control group, the difference was statistically significant（P〈0.05）. The length of hospital stay, operation time and blood loss in experiment group were significantly less than those in control group, the difference was statistically significant（P〈0.01）. The functional recovery of patients in experiment group was significantly higher than control group, the difference was statistically significant（χ^2=10.86, P〈0.01）. Conclusion The effect of locking titanium plate non bone graft surgery in treatment of calcaneal fractures of Sanders type 2 and type 3 is significant. Patients recovere well after surgery. It is worthy of clinical extensive promotion.
