摘要
目的 :探讨地佐辛联合右美托咪定对食管癌手术患者麻醉恢复期躁动的影响。方法 :选取2012年9月至2015年1月行食管癌手术患者45例,随机分为A组、B组和C组各15例。A组在麻醉诱导前应用右美托咪定,B组在手术开始时应用地佐辛,C组联合应用右美托咪定和地佐辛。观察患者手术前和气管拔管后的血流动力学、恢复期躁动程度、Ramsay评分、不良反应发生情况。结果 :在气管拔管后5、10、20 min时,C组患者收缩压、舒张压和心率均低于A组和B组(P<0.05)。C组躁动发生率为6.67%(1/15),低于A组的40.00%(6/15)和B组的46.67%(7/15,P<0.05)。C组Ramsay评分为(3.09±0.44)分,高于A组的(2.19±0.25)分和B组的(2.28±0.36)分(P<0.05)。三组患者在恢复期均未发生过度镇静、呼吸抑制和恶心呕吐等情况。结论 :地佐辛联合右美托咪定对食管癌手术患者麻醉恢复期躁动改善较好,临床应用价值高。
Objective： To explore the influence of dezocine combine with dexmedetomidine on restlessness of patients with esophageal carcinoma in anesthesia recovery period after surgical operation. Methods： Forty-five cases of esophageal carcinoma undergone surgical operation were selected and divided into group A, group B and group C with 15 patients each. Patients in group A were received dexmedetomidine before anesthesia induction. Patients in group were received dezocine at the beginning of operation. And patients in group C were received bothe dexmedetomidine and dezocine. The hemodynamics, degree of restlessness during recovery period, Ramsay score and adverse reaction of the patients before and after tracheal extubation were observed. Results： After 5, 10, 20 min of extubation, the systolic blood pressure, diastolic blood pressure and heart rate were lower in group C than in group A and group B（P〈0.05）. The incidence of agitation in group C was 6.67%（1/15）, which was lower than that in group A（40.00%,6/15）and group B（46.67%, 7/15, P〈0.05）. The Ramsay score was higher in group C（3.09±0.44）than that in group A（2.19±0.25）and group B（2.28±0.36, P〈0.05）. No excessive sedation, respiratory depression, nausea and vomiting occurred in the three groups during the recovery period. Conclusion： The combination of dezocine and dexmedetomidine in surgical operation can improve the restlessness of patients with esophageal cancer in anesthesia recovery period, and the clinical application value is high.
