摘要
目的分析总结先天性单一冠状动脉(SCA)的CT表现,提高对该畸形影像学表现的认识。方法回顾性分析25例经CT冠状动脉造影(CTA)诊断为SCA患者资料,使用第二代双源CT多种后处理方法观察冠状动脉变异情况,并与冠状动脉造影(CAG)结果进行对照分析。结果 25例SCA患者中,左冠状动脉缺如单右冠状动脉畸形2例,右冠状动脉缺如单左冠状动脉畸形23例,且所有病例均通过多平面重组(MPR)、最大强度投影(MIP)和容积再现(VR)等显示异常血管的形态。其中8例患者在CTA检查后行CAG检查,结果证实为SCA,CTA对其检出率为100%。结论双源CT血管成像可以准确显示正常和畸形的冠状动脉,清晰表现单一冠状动脉走行分布,可以作为诊断SCA的首选检查方法。SCA特征性CTA表现,可为临床早期干预和手术治疗提供可靠的影像学证据。
Objective To investigate the CTA findings of congenital malformation of the Single coronary artery( SCA),and to highlight the awareness of this malformation. Methods Dual-source CTA of patients who were diagnosed as congenital malformation of SCA between January 2011 to January 2016 were retrospectively analyzed. Using dual source CT various postprocessing methods to observe the variation of the coronary artery were performed and compared with the results of coronary angiography( CAG). Results In 25 cases of SCA,2 were diagnosed with the absence of left coronary artery single right coronary artery malformation and 23 were diagnosed with absence of right coronary artery single left coronary artery malformation,and all cases were reconstructed by MPR,MIP and VR showed abnormal vascular morphology. Among them,8 patients were examined by coronary angiography( CAG) after CTA examination,the results confirmed that the single coronary artery malformation,and the CTA detection rate was 100%. Conclusion Dual source CT angiography can clearly display the normal and variant changes and clearly show the morphology of SCA,which can be used as the first choice for diagnosis of SCA. SCA characteristic CTA performance can provide reliable imaging evidence for early intervention and surgical treatment.
