摘要
目的探讨氯沙坦对慢性肺心病心力衰竭患者右心室Tei指数与血浆N-末端脑钠肽前体（NT-proBNP）水平的影响。方法慢性肺心病心力衰竭患者120例，随机分为两组，即氯沙坦组（60例）和对照组（60例）。对照组仅接受常规治疗，氯沙坦组在常规治疗基础上给予氯沙坦50mg／d。检测两组治疗前及治疗6个月后右心室Tei指数与血浆NT-pro-BNP水平变化。结果肺心病心力衰竭患者右心室Tei指数与血浆NT-oproBNP水平呈正相关。治疗6个月后，两组右心室Tei指数、NT-pro—BNP水平较治疗前均有所下降，差异有统计学意义（P〈0．05，P〈0．01），但氯沙坦组下降更为显著，两组比较差异有统计学意义（P〈0．01）。结论氯沙坦能有效降低右心室Tei指数和血浆NT-pro-BNP水平，改善肺心病患者的心功能，这与氯沙坦能阻抑心室重构，改善心肌顺应性有关。
Objective Explore the effect of Losartan on right ventricular Tei index and N-terminal brain natriuretic peptide precursor （ NT- proBNP ） in patients with Chronic cor pulmonale heart failure. Methods 120 patients with chronic cot pulmonale heart failure were randomly divided into two groups： the Losartan group （ 60 cases ） and the control group （ 60 cases ） . The control group only accepted routine treatment, and Losartan group on the basis of routine therapy for Losartan 50 mg/d. The Tei index and NT-pro-BNP were detected in the two groups before and after 6 months. Result The right ventricular Tei index were positively correlated with the level of NT-proBNP in patients with chronic cot pulmonale heart failure. The Tel index and the NT-proBNP level were significantly decreased 6 months after treatment than those before treatment in two groups （ P〈0.05, P〈0.01 ） , but the Losartan group decreased more significantly.The difference between the two groups was statistically significant （ P〈0.01 ） . Conclusions Losartan can effectively reduce the fight ventricular Tei index and NT-proBNP levels, and improve heart function in patients with Chronic cot pulmonale, this may be associated with Losartan obstructing the ventricular remodeling and improving myocardial compliance.
