摘要
目的:对某院围手术期患者的肠外营养(TPN)处方进行分析,对临床药师干预前后的效果进行比较,为进一步规范TPN的临床应用提供参考。方法:采用回顾性分析的方法,根据《临床诊疗指南-肠外肠内营养学分册》(2008年版)的标准,对某院普通外科2014年7月至12月和2015年同期的TPN处方进行归纳和分析,具体包括营养素的组成及葡萄糖和氨基酸所占比例、糖脂比、热氮比、电解质浓度、谷氨酰胺的使用情况,并对临床药师干预前后的效果进行比较。结果:该院普通外科TPN处方存在不足,主要表现在糖脂比、热氮比、谷氨酰胺的使用方面等,在临床药师的干预下,合格率有所提高。结论:临床药师在保证TPN组方的配伍稳定性和合理的配比方面了发挥重要的作用。
OBJECTIVE To provide a reference for further standardizing the rational use of TPN in clinical practice by,we investigated and analyzinged the rationality of total parenteral nutrition(TPN)prescriptions and comparingdid comparison effects before and afterbetween period of pharmacist intervention.METHODS According to the requirements of Clinical Diagnosis and Treatment Guideline,Parenteral and Enteral Nutrition Fascicule(2008 edition),and also by rretrospective analysis method as well,the rationality ofin 2 826 TPN prescriptions in the general surgery department of our hospital was analyzed from July to December 2014,were performed in investigatio in on the terms of liquid the amountvolume of liquid,ratio of glucose/lipid ratio,ratioof non-protein/nitrogen energy ratio,amount of alanyl-glutamine and related factors of TPN stability.Comparison would been done during the process.RESULTS The unreasonable prescriptions existed in the ratio of glucose to lipid ratio,ratio of non-protein energy to nitrogen ratio,and amount of alanyl-glutamine among the partial TPN prescriptions.Percent of pass was significantly improved after pharmacist intervention.CONCLUSION Is vital effectsrole that clinical pharmacist play in compatibility stability of TPN prescriptions.
