摘要
目的探讨影响人血清附睾蛋白4(Human epididymis protein 4,HE4)诊断卵巢癌的相关因素。方法依据临床诊断和手术病理检查结果,收集168例卵巢癌患者,212例健康体检女性,104例妇科良性疾病患者,105例肾脏疾病患者,38例非肾脏疾病患者及75例恶性肿瘤患者的临床资料和标本。采用罗氏电化学发光法测定患者外周血HE4水平,采用SPSS 19.0软件进行统计学分析,探讨HE4诊断卵巢癌的影响因素。结果卵巢癌组血清HE4水平明显高于正常对照组、妇科良性疾病组、非肾脏疾病患者组以及其他恶性肿瘤组(P<0.05)。在肾脏疾病组中,血清HE4水平随慢性肾脏病分期逐渐增高,CKD4和CKD5期患者血清HE4水平高于卵巢癌组(P<0.001)。卵巢癌组血清HE4阳性率为77.4%,在未考虑慢性肾脏病下HE4对卵巢癌有较佳的诊断价值。同时年龄、FIGO分期、是否绝经、生育史、家族史对卵巢癌血清HE4水平有影响(P<0.05)。结论临床采用HE4诊断卵巢癌时,除了考虑年龄、FIGO分期、是否绝经、生育史以及家族史对HE4水平造成影响外,还应特别注意肾脏疾病对检测结果的影响。
Objective To explore the related influencing factors of diagnosis of ovarian cancer by human epididymis protein 4( HE4).Methods According to clinical diagnosis and pathological results after surgery,the clinical data and specimens of 168 patients with ovarian cancer,212 healthy women,104 patients with benign gynecologic diseases,105 patients with kidney diseases,38 patients with other diseases except for kidney diseases,and 75 patients with malignant tumors were collected. Roche electrochemical luminescence method was used to detect HE4 levels in peripheral blood. SPSS 19. 0 software was used for statistical analysis,the influencing factors of diagnosis of ovarian cancer by HE4 were analyzed. Results The level of serum HE4 in ovarian cancer group was statistically significantly higher than those in normal control group,benign gynecologic disease group,non-kidney disease group and other malignant tumor group( P〈0. 05). In kidney disease group,serum HE4 level increased gradually with the staging of chronic kidney diseases,the levels of serum HE4 in patients of stage CKD4 and CKD5 were higher than that in ovarian cancer group( P〈0. 001). The positive rate of serum HE4 in ovarian cancer group was77. 4%,the diagnostic value of HE4 for ovarian cancer was good excluding the impact of chronic kidney diseases. Age,FIGO staging,menopause or not,childbearing history and family disease history had significant impacts on serum HE4 level in ovarian cancer patients( P 0. 05). Conclusion The impact of chronic kidney diseases should be considered in diagnosis of ovarian cancer using HE4 besides age,FIGO staging,menopause or not,childbearing history and family disease history.
