摘要
目的分析营养膳食护理对老年骨折术后营养状态、免疫功能及切口愈合的影响。方法 2013年11月~2015年11月在我院接受治疗的老年骨折患者90例,按照随机数字表法分为对照组(n=45)及观察组(n=45),对照组患者接受常规护理,观察组患者在常规护理基础上加入营养膳食护理。干预后,检测两组患者的外周血蛋白指标、T淋巴细胞亚群及免疫球蛋白水平,记录切口干燥时间、引流物放置时间、切口愈合时间,以及切口皮肤裂开、切口感染、切口皮肤坏死等不良愈合事件发生比例。结果干预后,观察组患者的外周血蛋白指标白蛋白(Alb)、前白蛋白(PA)、转铁蛋白(TF)、血红蛋白(Hb)含量均显著高于对照组患者(P<0.05);观察组患者的外周血CD4+T淋巴细胞水平及CD4+/CD8+比值高于对照组患者,CD8+T淋巴细胞水平低于对照组患者,免疫球蛋白G(Ig G)、免疫球蛋白A(Ig A)、免疫球蛋白M(Ig M)含量高于对照组患者(P<0.05);观察组患者的切口干燥时间、引流物放置时间、切口愈合时间少于对照组患者,切口皮肤裂开、切口感染、切口皮肤坏死比例均低于对照组患者(P<0.05)。结论营养膳食护理可优化老年骨折患者的术后营养状态及免疫功能,促进切口愈合。
Objective To study the influence of diet and nutrition nursing on nutritional status, immune function and wound healing in senile fractures patients. Methods Ninety senile fractures patients in our hospital during November 2013-November 2015 were divided into control group (n=45) and observation group (n=45) according to random data form, patients in control group received routine nursing care and those in observation group received Diet and Nutrition nursing plus routine nursing care. After intervention, peripheral blood protein indexs, T lymphocyte subsets and immunoglobulin levels in two group were detected, wound drying time, drainage time, wound healing time and incidence rates of adverse event such as skin incision dehiscence, wound infection, incision skin necrosis were recorded. Results After intervention, protein index of peripheral blood Alb, PA, TF, Hb were significantly higher in observation group than in control group (P〈0.05); the observation group of patients with peripheral blood CD4+T lymphocytes and CD4+/ CD8+ ratio were higher in observation group than in control group, CD8+T lymphocyte level was lower in observation group than in control grouP, immunoglobulin IgG IgA and IgM were higher in observation group than in control group (P〈0.05); wound drying time, drainage time, wound healing time were less in observation group than in control group, incidence rates of adverse event such as skin incision dehiscence, wound infection, incision skin necrosis were lower in observation group than in control group (P〈0.05). Conclusion Diet and nutrition nursing can optimize the nutritional status and immune function in senile fractures patients, promote the quality of incision healing.
