玻璃体切除联合改良睫状体固定术治疗复杂的睫状体脱离 被引量:4

Vitrectomy combined with improved cyclopexy for the treatment of complicated cyclodialysis clefts

摘要 目的观察玻璃体切除联合改良睫状体固定术治疗复杂的外伤性睫状体脱离的效果。方法回顾性分析我院收治的10例(10眼)复杂的睫状体脱离临床资料。所有患者术前行视力检查、裂隙灯显微镜、非接触眼压计、前房角镜、超声生物显微镜(UBM)及眼B超检查。术中均行平坦部玻璃体切除联合改良睫状体固定术。术后随访视力、眼压和睫状体复位情况。结果术后随访时间6~14个月,平均(9.50±2.68)个月;术后所有患者视力均有改善;术前眼压平均(4.10±1.91)mmHg(1mmHg=0.133kPa),术后眼压均恢复到正常范围,平均(13.00±3.02)mmHg,差异有统计学意义(t=10.03,P〈0.05);术前UBM检查示10只眼睫状体脱离范围1—5个钟点位,平均(2.30±1.34)个钟点位,术后检查示睫状体均成功复位。结论玻璃体切除联合改良睫状体固定术治疗复杂的睫状体脱离效果确切,是一种简便、安全的手术方式。 Objective To observe the efficacy of vitrectomy combined with improved cyclopexy for complicated cyclodialysis clefts. Methods Ten eyes of 10 patients with complicated cyclodialysis clefts were enrolled in the retrospective study. Preoperatively, the eyes were examined with slit lamp, ultrasound biomicroscope (UBM) and B-ultrasound. The visual acuity (BCVA) and intraocular pressure (IOP) were measured preoperatively. All patients underwent vitrectomy with improved direct cyclopexy. The BCVA, IOP and cyclodialysis clefts repairment were followed up. Results The followed up time was 6 - 14 months, average (9.50 ±2.68) months. The BCVA was improved in all patients after surgery. Preoperatively, the mean IOP was (4.10 ± 1.91 ) mmHg. The mean postoperative IOP was ( 13. 00 ±3.02) mmHg, which was significantly improved compared with the preoperative IOP ( P 〈 0.05 ). The UBM results before surgery showed that cyclodialysis clefts in 10 eyes were about 1 - 5 clocks w/th the average walue (2.30 ±1.34) clocks. And cyclodialysis clefts were completely closed in 10 patients postoperatively. Conclusion Vitrectomy combined with improved cyclopexy is an effective, convenient and safe method to treat complicated cyclodialysis clefts.
