摘要
目的了解经方治疗神经根型颈椎病(CR)的临床应用现状。方法检索中国知网、Pub Med数据库,纳入经方治疗CR的病例系列报告或者是随机对照试验,检索时限为2000年1月1日至2016年3月31日。分别由两名作者独立检索、筛选文献,利用note Express 2.0软件进行文献管理。结果 21篇文献符合纳入标准,其中桂枝加葛根汤、葛根汤、柴胡桂枝汤、黄芪桂枝五物汤、当归四逆汤、乌头汤是CR治疗常用方剂。临床实践中常在经方基础上进行加减,以方测证,虚、风(寒)、瘀是CR的主要病机特点。经方治疗CR的随机对照试验质量较差,存在较高程度的偏倚。结论传承经方精髓是经方临床应用与创新研究的前提,临床治疗CR应从虚、风(寒)、瘀论治,同时要提高研究质量。鉴于经方治疗CR的中药新药研究较少,未来可开展相关工作。
Objective To understand the present situation of the clinical application of the classic formulas in treatment of cervical radiculopathy（CR）. Methods After retrieving from CNKI and Pub Med,the case reports or RCTs were included in the treatment of CR with classic formulas,focusing the literatures from January 1,2000 to March 31,2016. Two authors were responsible for the independent retrieval and literature screening. Note Expression 2. 0 was used for literature management. Results Totally 21 literatures were in compliance with the inclusive criteria,in which,guizhijiagegen tang,gegen tang,chaihuguizhi tang,huangqiguizhiwuwu tang,dangguisini tang and wutou tang were the common formulas in the treatment of CR. In clinical practice,the classic formulas were used with modification and detect the patterns/syndromes. Deficiency,wind（cold） and stasis were the main pathogenesis. The quality of RCT for CR treatment was poor with high bias. Conclusion The inheritance of classic formulas is the prerequisite for the clinical application and innovative study. The clinical treatment of CR should be on the basis of deficiency,wind（cold） and stasis and improve the study quality. Concerning the less new drugs of TCM used in CR treatment,the relevant work should be carried on in the future.
