摘要
目的探讨高压氧(HBO)治疗对皮瓣移植术后早期干预的价值。方法选择我科2011年6月~2015年4月行皮瓣移植术的60例患者作为研究对象,根据治疗方法不同分为两组,对照组30例患者进行皮瓣移植术后均予抗凝、解痉、抗感染等基础治疗,同时采取保温、减张等措施。观察组30例患者在此基础上观察组辅助HBO治疗。观察两组皮瓣经皮血氧饱和度、皮温及存活率。结果观察组患者术后皮瓣移植区经皮血氧饱和度和皮温均明显高于对照组(P<0.05);观察组移植皮瓣存活率明显高于对照组(93.4%vs.73.3%,P<0.05);术后48 h内行HBO治疗,皮瓣存活率为100.0%,明显高于术后48 h的存活率(71.4%)(P<0.05)。结论皮瓣移植术后早期行HBO治疗可提高移植皮瓣成活率,术后48 h内应用为最佳介入时机。
Objective To investigate the clinical value of HBO treatment on skin flap after transplantation with early intervention.Methods 60 cases with skin flap transplantation operation treated in our department from June 2011 to April 2015 were selected as the research objects,and they were divided into two groups according to different thera- peutic methods.30 patients were treated with additional HBO after transplantation of skin flaps and 30 patients as control were treated with conventional method alone.Flap transeutaneous oxygen saturation,temperature and flap survival rate were measured in two groups.Results The flap transcutaneous oxygen saturation and temperature in observation group were significant higher than those in control group （P〈0.05）.The flap survival rate of observation group was obviously higher than the eontrol group （93.4% vs.73.3%,P〈0.05）.HBO treatment was conducted within 48 h after operation, the flap survival rate was 100.0%,and it ws significantly higher than that after 48 h operation （71,4%）（P〈0.05）.Conclusion Treatment of HBO has been done early after operation can improve survival rate of transplanted skin flap,HBO treatment conducted within 48 h after operation is the best intervention time.
