摘要
目的探讨康复护理措施对老年慢性阻塞性肺疾病(COPD)患者肺功能和生活质量的影响。方法选择2015年1月~2016年6月于我院呼吸内科接受治疗的COPD患者120例纳入本研究,根据入院时间编号后将其分为对照组和观察组,各60例。对照组给予常规治疗和护理,观察组采取康复护理措施,比较两组患者干预前后肺功能指标和生活质量变化。结果观察组患者干预后肺功能指标FVC、FEV1、FVC/FEV1及PEF、MMEF均较干预前明显改善(P<0.05),且与对照组比较差异有统计学意义(P<0.05)。观察组患者干预后6 min步行距离较干预前明显增加(P<0.05),且与对照组比较差异有统计学意义(P<0.05)。观察组患者干预后SGRO问卷调查中呼吸症状、活动受限及疾病影响等维度评分,均较干预前明显减少(P<0.05),且与对照组比较差异有统计学意义(P<0.05)。结论老年COPD患者实施康复护理措施能改善其肺功能,增强运动耐力,提高生活质量,在临床应用中极具推广价值。
Objective To explore the effect of rehabilitation nursing on pulmonary functions and quality of life in elderly patients with chronic obstructive pulmonary disease （COPD）.nethods Altogether 120 cases of COPD patients who were treated in our hospital from January 2015 to June 2016 in Department of Respiratory Medicinewere involved in this study and divided into control group and observation group according to the admission time number,60 cases in each group.The control group was treated with routine therapy and nursing;while,the observation group was adopted rehabili- tation nursing aneasures.The changes of lung functions and quality of life were compared between the two groups.Results Post-intervention pulmonary function indexes such as FVC,FEV1,FVC/FEV1,PEFand MMEF of the patients in observation group were significantly improved than those before intervention（P〈0.05）,compared with control group,the differences were significant （P〈0.05）.Post-intervention 6 MWT of patients in observation group was significantly increased than that before intervention（P〈0.05）,compared with control group,the difference was significant （P〈0.05）.Post-intervention dimension scores of respiratory symptoms,activity limitation and the impact of disease in SGRQ of patients in observation group were significantly reduced （P〈0.05） in contrast with those before intervention and had statistically significant difference compared with those of control group （P〈0105）.Conclusion Rehabilitation nursing measures can improve the lung functions,enhance exercise tolerance and improve the quality of life in elderly patients with COPD,which is of great value in clinical application.
