摘要
目的探讨二维、三维超声对胎儿先天性心血管畸形筛查作用。方法采用随机抽样与回顾性研究方法,2013年1月至2016年4月选择在我院诊治的35名疑似胎儿心血管畸形的产妇作为研究对象,所有产妇都给予二维、三维超声,然后与病理检查结果进行对比分析,判断诊断准确性。结果在35名产妇胎儿中,病理检查结果显示出102处心脏异常情况,其中房室间隔异常35处、动脉心室连接异常28处、房室连接异常25处、动脉干异常14处。二维联合三维超声对各种类型心血管畸形的正确诊断率都高于二维超声(P〈0.05)。二维超声检查对胎儿先天性心血管畸形的诊断敏感度、特异度、阳性预测值及阴性预测值分别为84.9%、50.0%、96.6%和16.7%,而二维联合三维超声检查分别为97.0%、100.0%、100.0%和66.7%,二维联合三维超声检查的敏感度、特异度、阳性预测值及阴性预测值都高于二维超声(P〈0.05)。结论超声检查在胎儿先天性心血管畸形诊断中具有重要的意义。二维联合三维超声检查有利于对心脏形态学进行客观评估,提高诊断效果,有很好的应用价值。
Objective To investigate the effects of two-dimensional and three-dimensional ultrasound in screening fetal congenital cardiovascular malformation and improve the diagnostic efficiency. Methods Used the random sampling and retrospective study method, from January 2013 to April 2016, 35 women of suspected fetal cardiovascular malformations in our hospital for treatment were selected as the research object, all women were giving the two-dimensional, three-dimensional ultrasound and pathological examination that the results were compared and analyzed, and were given to determine the effectiveness of diagnosis. Results In the 35 women, pathological examination revealed 102 abnormal cardiac conditions that 35 were abnormal atrioventricular septal, 28 were ab- normal ventricular connections, 25 were abnormal atrioventricular connections, 14 were abnormal arterial trunks. However, the correct diagnosis rate of various types of cardiovascular malformations by the two-dimensional ultra- sound combined the three-dimensional ultrasound were higher than that of two-dimensional ultrasound (P〈0.05). And the diagnosis sensitive, specificity, positive predictive value and negative predictive value of fetal congenital cardiovascular malformation by the two-dimensional ultrasonography were 84.9%, 50.0%, 96.6% and 16.7%, and were 97.0%, 100.0%, 100.0% and 66.7% by the two-dimensional ultrasound combined the three-dimensional ul- trasound that compared were all significantly difference(P〈0.05). Conclusion Ultrasonograpby in diagnosis of fetal congenital cardiovascular malformation has important significance, two-dimensional ultrasound combined the three-dimensional ultrasound has an objective assessment of cardiac morphology, it can improve the level of diagnosis that has good application values.
