摘要
目的探讨老年冠心病患者血清B型利钠肽(BNP)和高敏C反应蛋白(hs-CRP)的水平及临床意义。方法选取住院的66例冠心病患者作为研究组(不稳定性心绞痛者43例,稳定性心绞痛者23例),同时选取同一时期住院的50例非冠心病患者作为对照组,所有患者均检测血浆纤维蛋白(Fn)、血清BNP以及hs-CRP水平,对所有患者进行为期1年的随访,比较两组患者心血管不良事件发生率。结果研究组血浆Fn与对照组比较[(401.33±112.14)mg/ml比(525.46±115.92)mg/ml]差异具有统计学意义(P<0.01);研究组患者其血清BNP和hs-CRP水平均显著高于对照组(均为P<0.01);其中不稳定性心绞痛患者血清BNP[(409.33±47.14)mg/ml比(32.34±8.62)mg/ml]和hs-CRP[(1.58±1.12)mg/ml比(0.99±0.35)mg/ml]水平显著高于稳定组患者(均为P<0.05);随访1年后发现有13例出现不良事件,且入院时不良事件发生者血清BNP[(372.12±24.80)mg/ml]明显高于未发生者[(100.63±17.6)mg/ml]。结论血清BNP和hs-CRP在不同冠心病患者中水平存在明显差异,在预测冠心病预后情况上具有重要价值。
Objective To explore serum levels of B-type natriuretic peptide (BNP) and high sensitivity C-reactive protein (hs-CRP)in elderly patients with coronary heart disease and its clinical significance. Methods A total of 66 patients with coronary heart disease were selected as study group(43 patients with unstable angina pectoris and 23 patients with stable angina pectoris). Fifty patients without coronary heart disease were selected as the control group. All the patients were tested for plasma fibrin( Fn), serum BNP and hs-CRP. All patients were followed up for 1 year. The incidence of adverse cardiovascular events was compared between the two groups. Results The difference of plasma Fn between the two groups [ (401.33± 112. 14) mg/ml vs. (525.46 ± 115.92) mg/ml] was statistically significant(P 〈0. 01 ). The levels of serum BNP and hs-CRP in patients with unstable angina were significantly higher than those in the control group( P 〈 0. 01 ). The serum BNP [ (409. 33 ± 47. 14 ) mg/ml vs. ( 32. 34 ± 8.62 ) mg/ml ] and hs-CRP [ ( 1.58 ± 1.12 ) mg/ml vs. (0. 99 ± 0. 35 ) mg/ml ] of unstable angina patients were significantly higher than those of the stable group ( all P 〈 0. 05 ). The serum BNP [ ( 372. 12 ± 24. 80 ) mg/ml ] was significantly higher than that of the untreated patients [ ( 100. 63 ± 17.6) mg/ml]. Conclusions Serum BNP and hs-CRP have prognostic value in elderly patients with coronary heart disease.
