磁共振弥散加权成像在前列腺中央腺体癌早期诊断中的价值 被引量:6

The value of magnetic resonance diffusion weighted imaging in early diagnosis for central gland prostate cancer

摘要 目的探讨磁共振扩散加权成像(diffusion-weighted imaging, DWI)及表观扩散系数(apparent diffusion coefficient,ADC)诊断及鉴别早期前列腺中央腺体癌(central gland prostate cancer, CGPCa)的价值。方法选择25例早期CGPCa和30例良性前列腺增生(benign prostate heperplasia, BPH)患者进行常规MRI和DWI检查,依据病灶DWI检查后的图像测定ADC值,并采用受试者工作特征(ROC)曲线分析,以确定诊断CGPCa的最佳ADC值。结果CGPCa的ADC值[(0.89±0.12)×10^-3 mm2/s]明显低于BPH的ADC值[(1.43±0.12)×10^-3 mm^2/s],差异有统计学意义(P〈0.05)。当ADC临界值设为1.192×10^-3 mm^2/s时,敏感性为80.0%,特异性85.0%。结论DWI及ADC在检出及鉴别CGPCa的诊断中有临床应用价值。 ObjectiveTo investigate the value of magnetic resonance diffusion weighted imaging (DWI) and apparent diffusion coefficient (ADC) in detection of central gland prostate cancer (CGPCa).MethodsRoutine MRI and DWI examination were performed in 25 CGPCa patients and 30 benign prostate heperplasia (BPH) patients. Then the ADC value was measured according to the image of the lesion after DWI, and the receiver operating characteristic (ROC) curve was analyzed to select the best ADC value for diagnosis of CGPCa.ResultsThe ADC value of CGPCa (0.89±0.12×10-3 mm2/s) was smaller than that of BHP (1.43±0.12×10-3 mm2/s) , and the difference was statistically significant (P〈0.05) . When the cut off point of ADC value was set at 1.192×10-3 mm2/s, the sensitivity was 80.0% and the specificity was 85.0%.ConclusionDWI and ADC have clinical application value in diagnosis of CGPCa.
