摘要
目的研究阿司匹林相关性胃黏膜损害患者中.胃上皮细胞紧密连接蛋白Occludin表达的变化及上皮细胞间隙的改变。方法采用免疫组织化学法检测70例因服用阿司匹林导致胃糜烂或胃溃疡患者(其中胃糜烂45例、胃溃疡25例)及15例正常对照胃黏膜上皮细胞紧密连接蛋白Occludin的表达,光镜下观察切片,采用图像分析系统对每例切片测量50个细胞间隙,进行统计学分析。结果 15例正常对照组Occludin呈正常连续的细胞膜表达;胃糜烂组10例正常表达,35例不同程度异常表达,主要为细胞膜不完整表达、细胞质表达及表达缺失;25例胃溃疡均表达异常。胃糜烂组和胃溃疡组Occludin异常表达阳性率与正常对照组比较,差异均有统计学意义(P<0.01)。而胃糜烂组和胃溃疡组组间比较,差异无统计学意义(P>0.05。胃糜烂组和胃溃疡组平均细胞间隙与正常对照组比较,差异均有统计学意义(P<0.01),而胃糜烂组和胃溃疡组对比(P>0.05。结论阿司匹林相关性胃黏膜损害患者胃黏膜上皮Occludin分布异常、数量减少、细胞间隙增宽,影响黏膜上皮屏障的完整性,导致胃黏膜损害。
Objective To investigate the expression of Occludin in gastric epithelial cells in patients with aspirin associated gastric mucosal lesion and the changes of the intercellular space. Methods 70 cases with aspirin can cause stomach erosion or ulcer patients by immunohistochemical method （ including 45 cases of gastric erosion, 25 cases of gastric ulcer ） expression and 15 cases of normal g^stric mucosa epithelial tight junction protein Occludin. The samples were observed under light microscope, and 50 cell gaps were measured by image analysis system. Results The expression of Occludin in 15 cases of normal control group was normal. Normal expression in 10 cases of gastric erosion group, 35 cases of abnormal expression. The main factors were incomplete expression of cell membrane, cytoplasmic expression and deletion; 25 cases of gastric ulcer were abnormal. The positive rate of Occludin expression in gastric ulcer group and gastric ulcer group was significantly higher than that in normal control group （ P 〈 0.01 ） . However, there was no significant difference between the gastric erosion group and the gastric ulcer group （P 〉 0.05 ） . Compared with normal control group, there was a significant difference in mean intercellular space between gastric erosion group and gastric ulcer group （ P 〈 0.01 ）, compared with gastric ulcer group and gastric tdcer group （ P 〉 0.05 ） . Conclusion The Occludin distribution of gastric mucosa in patients with aspirin associated gastric mucosal lesion was decreased, the number of cells decreased and the intercellular space widened, affect the integrity of the mucosal barrier, gastric mucosa damage.
