摘要
目的:评价血必净注射液对慢性阻塞性肺疾病急性加重期(AECOPD)患者的临床肺部感染评分(Clinical pulmonary infection score,CPIS)、炎症标记物及免疫功能的影响以及其临床疗效。方法:将78例AECOPD患者随机分为两组(治疗组和对照组),每组均为39例,两组均予常规抗炎、解痉平喘、祛痰治疗,治疗组加用生理盐水250 m L+血必净注射液100 m L 1次/d静脉滴注,疗程为10 d。观察两组的临床疗效、记录治疗前后两组患者的CPIS评分并检测患者血清中炎症标记物(C反应蛋白CRP、降钙素原PCT、白细胞总数WBC)及免疫功能(CD_3、CD_4、CD_8、CD_4/CD_8)的变化。结果:治疗后,治疗组与对照组患者所有观察指标均较治疗前明显改善(P<0.05)。治疗后除白细胞总数差异无统计学意义外(P>0.05),治疗组在治疗后总有效率、CRP、PCT、CD_3、CD_4、CD_8、CD_4/CD_8均明显优于对照组,且差异具有统计学意义(P<0.05)。结论:临床应用血必净注射液协同治疗AECOPD,能提高患者免疫功能,减轻炎症反应,明显提高治疗有效率。
Objective :To evaluate the clinical effects of Xuebijing Injection on CPIS scores and inflammatory markers and inmmne function of patients with AECOPD. Methods : A total of 78 patients with AECOPD were randomly divided into two groups :38 cases in the treatment group and 38 cases in the control group. All patients received the conventional treat- ments including anti - infection, antispasmodic, anti - asthmati, expeetoranttherapyand 100mL Xuebijing Injection with NS 250mL(once a day, ten days for a course) were added in the treatment group. The changes of serum CD3 , CD4 , CD8, CD4/CD8 and CRP,PCT and WBC were tested. The curative effect and clinical pulmonary infection scores were observed for ten days. Result : After the full course of treatment, the treatment group and the control group' s indicatorswere all ob- served and significantly improved ( P 〈 0. 05 ). After treatment, there was no significant difference in the total white blood cell count (P 〉 0. 05). After treatment, the total effective rates, such asCRP, PCT, CD3 , CD4 , CDs and CDo/CDs were significantly better than the control group' s, showing significant difference ( P 〈 O. 05 ). Conclusion : Clinical application of Xuebijing Injection synergistic treatment of AECOPDcan improve the immune function of patients, reduce inflammation and significantly improve the efficiency of treatment.
