摘要
目的探讨椎间孔针闭合松解椎间孔外口结合术后口服藤黄健骨胶囊治疗腰椎间盘突出症的临床疗效。方法选取2014年8月~2016年10月我院骨科收治的腰椎间盘突出症患者174例,按随机数字表法分组,对照组87例予以椎间孔闭合松解术治疗,研究组87例在对照组基础上予以藤黄健骨胶囊治疗,观察并记录两组间腰椎症状及功能评分、血清疼痛物质水平、血清炎性因子水平,同时比较临床疗效。结果总有效率对照组(85.06%)低于研究组(95.40%),差异具有统计学意义(P<0.05);与对照组比较,研究组治疗后VAS、ODI评分较低,JOA评分较高,治疗后血清SP、NGF、NPY水平较低,治疗后血清TNF-α、PCT、IFN-γ水平较低,差异具有统计学意义(P<0.05)。结论椎间孔闭合松解椎间孔外口结合术后口服藤黄健骨胶囊治疗腰椎间盘突出症的临床疗效确切,能够明显降低血清疼痛物质及炎性因子水平,减轻患者腰腿疼痛症状及炎症反应,抑制病情发展,缓解腰椎功能障碍。
Objective To study the clinical effect of the intervertebral foramen needle closed lysis at the intervertebral foramen opening combined with postoperative oral administration of Tenghuang jiangu capsule？in the treatment of lumbar disc herniation. Methods 174 patients with lumbar disc herniation admitted in the Department of Orthopedics from August 2014 to October 2016 were chosen and divide,d into two groups according to the randon number table methQd. 87 cases in the control group were treated with intervertebral forameration closed lysis. And the 87 patients in the study group were given Tenghuang jiangu capsule on the basis of the treatment of the study group. The lumbar vertebrae symptoms and function score, serum pain material level and serum inflammatory cytokine level were observed and recorded. The clinical efficacy was also compared. Results The total effective rate（85.06%） in the control group was lower than that in the study group（95.40%）, the difference was statistically significant（P〈0.05）. The VAS and ODI scores were lower and JOA score was higher than those of the control group. Compared with those in the control group, the levels of serum TNF-α, PCT and IFN-γwere lower after treatment, and the levels of serum SP, NGF and NPY were lower, and the difference was statistically significant（P〈0.05）. Conclusion The intervertebral foramen needle closed lysis at the intervertebral foramen opening combined with postoperative oral administration of Tenghuang jiangu capsule in the treatment of lumbar disc herniation has exact treatment effect, which can significantly reduce the level of serum pain mate-ials and inflammatory factors, reduce the patient＇s lumbar and leg pain symptoms and !nflammatory response. It can also inhibit disease progression and relieve lumbar dysfunction.
