摘要
目的:观察大黄灌肠对重症急性胰腺炎(severe acute pancreatitis,SAP)患者血清高迁移率族蛋白B1(High mobility group B1,HMGB-1)与降钙素原(Procalcitonin PCT)的影响。方法:收集重症胰腺炎患者60例,随机分为大黄治疗观察组(30例)和常规治疗对照组(30例),分别在入院后第1、3、5天动态监测各组患者的HMGB1与PCT浓度变化。结果:治疗前两组患者HMGB-1、PCT水平差异均无统计学意义;治疗后5天,观察组患者HMGB-1、PCT水平较治疗前显著降低。Δ3-1HMGB1(第3天与第1天的HMGB1差值)与(acute physiology and chronic health evaluation system APACHEⅡ)评分呈明显负相关,与PCT呈明显正相关。结论:大黄通过有效抑制重症急性胰腺炎患者晚期炎症因子HMGB-1表达量,进一步能改善重症胰腺炎的症状和转归,为其治疗重症急性胰腺炎提供新的作用机制。
To observe the effect of rhubarb enema on serum high mobility group protein B1 (High mobility group B1,HMGB-1) and calcitonin (Procalcitonin PCT) in patients with severe acute pancreatitis (SAP). Methods:60 cases of severe pancreatitis were collected and randomly divided into rhubarb treatment group (30 cases) and routine treatment group (30 cases). Changes of HMGB1 and PCT concentration in each group were dynamically monitored at first,third,fifth day after admistration. Results: There was no significant difference in levels of HMGB-1 and PCT in two groups before the treatment (P >0. 05),and on the fifth day after the treatment,levels of HMGB-1 and PCT in the rhubarb group were significantly lower than those before the treatment,and the difference between two groups was statistically signifi-cant (P <0. 05). △3-1 HMGB1(the difference of HMGB1 between on third day and the first day) was negatively correlated with the score of(acute physiology and chronic health evaluation system APACHE II),and was positively correlated with PCT. Conclusions: Rhubarb can further improve symptoms and outcomes of severe acute pancreatitis by effectively inhibiting the expression of advanced inflammatory factor HMGB-1 in patients with severe acute pancreatitis,and the experiment provides a new mechanism for the treatment of severe acute pancreatitis
