摘要
目的观察复方佛耳草合剂对慢性阻塞性肺疾病急性加重(AECOPD)患者血清IL-8、TNF-α水平及外周血单核细胞Toll样受体(TLR2、TLR4)表达的影响。方法将120例AECOPD患者随机分为治疗组和对照组,每组60例。在常规西药治疗基础上,治疗组加服复方佛耳草合剂,对照组加服复方佛耳草合剂安慰剂。两组疗程均为14天,观察比较中医证候积分、血清炎症因子及外周血单核细胞Toll样受体(TLRs)水平的变化情况。结果 (1)最终完成试验者107例,治疗组51例,对照组56例。(2)组间治疗后比较,治疗组咳嗽、咯痰评分及总积分较对照组下降更加明显(P<0.05)。(3)组间治疗后比较,治疗组IL-8水平较对照组下降更加明显(P<0.05)。(4)组间治疗后比较,治疗组TLR2、TLR4水平较对照组升高更加明显(P<0.05)。(5)治疗前后血清炎症因子IL-8、TNF-α水平与外周血单核细胞TLR2、TLR4表达水平均具有良好的负相关性。(6)所有患者均未出现常规检查异常及严重不良反应。结论复方佛耳草合剂联合常规西药治疗AECOPD,可显著改善患者咳嗽、咯痰症状,可能与其减轻气道炎症反应、增强机体固有免疫应答有关。
Objective To observe the effects of Compound Fo＇ ercao Mixture on the serum levels of inflammatory factors （ IL-8, TNF-o~） and expressions of Toll like receptors（TLRs） in peripheral blood mononuelear cells of patients with acute exacerbation of chronic obstructive pulmonary disease（AECOPD）. Methods 120 patients with AECOPD were randomly divided into the treatment group and control group,60 cases in each group. Both groups were given the conventional therapy of western medicine. In addition, the treatment group was treated with Compound Fo＇ ereao Mixture, and the control group was treated with the placebo, with a course of t4 days. The changes of TCM syndrome scores ,serum levels of IL-8 ,TNF-c~ and expressions of TLR2 ,TLR4 in peripheral blood mononuclear ceils were observed and compared. Results （~Finally 107 cases completed the whole trial, among 51 cases in the treatment group and 56 cases in the control group. （~）After treatment, the decrease of the scores on cough,expectoration and the total score in the treatment group were more significant than those in the control group （P 〈 0.05 ）. （~）After treatment, the decrease of IL-8 level in the treatment group was more significant than that in the control group （ P 〈 0.05 ）. @After treatment, the increase of the expression levels of TLR2 and TLR4 in the treatment group were more significant than those in the control group（ P 〈 0.05 ）. （~）There were good negative correlations between the serum levels of IL-8, TNF-c~ and expression levels of TLR2 and TLR4 in peripheral blood mononuclear cells before and after treatment, t~）All patients had no abnormality in routine inspection and severe adverse reactions. Conclusion Compound Fo＇ ercao Mixture combined with conventional therapy of western medicine can significantly improve the main symptoms such as cough and expectoration in AECOPD patients, which may be associated with alleviating the airway inflammation and enhancing the innate immune response.
