摘要
目的探讨高频彩色多普勒超声对结节性筋膜炎(nodular fasciitis,NF)的诊断价值。方法回顾分析2010年1月至2015年5月在成都市第三人民医院诊治的17例经病理证实为NF患者的超声声像图资料,包括病变部位、大小、二维超声表现、毗邻关系及内部血流情况。结果 17例均表现为单发的梭形、椭圆形或浅分叶状结节,边界较清楚,部分结节后方可有不同程度增强,可有"筋膜尾"征。17例中有8例为低回声,5例高回声,混合性回声有4例,其中2例混合回声结节可伴部分液化。14例病灶血流信号不丰富,3例血流信号较丰富,PW可测及低速高阻动脉血流频谱。结节位于上肢8例,均位于屈侧,躯干5例,头颈部2例,下肢2例。7例位于皮下深筋膜,8例位于皮下浅筋膜,肌间、肌内各1例。病理分型:黏液型3例,细胞型4例,纤维型8例,含有两种类型及以上2例。结论 NF的声像图表现具有一定特征性,超声检查对于该病的诊断和鉴别诊断均具有一定临床价值。
Objective To explore the value of the high frequency ultrasound in the diagnosis of nodular fasciitis.Methods 17 cases treated in The Third People′s Hospital of Chengdu from January of 2010 to May of 2015 and confirmed pathologically as the patients with nodular fasciitis were selected,and their ultrasonic image data such as the lesion site,size,two-dimensional ultrasound imaging,adjacent relationship and internal blood flow were analyzed retrospectively.Results The data of all the 17 cases showed that the nodules were single withspindle,oval or lobulated shapes,the boundary was relatively clear,the rear parts of some nodules were enhanced to different extent,and there were fascia tail signs.There were 8cases with low echo levels,5cases with high echo levels,and 4cases with mixed echo levels among the17 cases.Besides,the nodules of the 2cases with mixed echo levels were accompanied with partial liquefaction.The blood flow signals of the lesions were not abundant in 14 cases,and they were relatively abundant in the other 3cases.PW and low speed high resistance arterial blood flow spectrum were obtained.The nodules of 8cases were located in the upper extremity,those of 5cases were in the lateral flexion or trunk,those of 2cases were in the neck,and those of the other 2cases were in the lower limbs.The nodules of 7cases were located in deep subcutaneous fascia,those of 8cases were in the subcutaneous superficial fascia,those of one case were intermuscular and those of the other one case were intramuscular.According to the pathological classification there were 3cases of mucous type,4cases of cell type,8cases of fiber type,and 2cases of 2types or even more.Conclusion The ultrasonic image of nodular fasciitis displays certain characteristics,and the ultrasonography is of certain clinical value in the diagnosis and differential diagnosis of the disease.
