摘要
目的:探讨泽泻汤辅助治疗肝硬化门静脉高压对患者血流动力学、肝纤维化指标的影响。方法:选取符合纳入标准的100例肝硬化门静脉高压患者采用随机数字表法分为治疗组和对照组各50例,两组均采用常规治疗,治疗组加用泽泻汤,疗程3个月。观察两组患者临床疗效,血流动力学及血清HA、PCⅢ、LN、Ⅳ-C水平的变化。结果:治疗3个月后,治疗组患者的门静脉血管内径、脾静脉血管内径、静脉血流量、脾静脉血流量均低于对照组,差异具有统计学意义(P<0.05),泽泻组的门静脉血流速度、脾静脉血流速度均高于对照组,差异具有统计学意义(P<0.05);泽泻组患者的血清HA、PCⅢ、LN、Ⅳ-C均低于对照组,差异具有统计学意义(P<0.05);泽泻组患者的总有效率92.00%高于对照组的76.00%,差异具有统计学意义(P<0.05)。结论:泽泻汤辅助治疗肝硬化门静脉高压能够显著改善患者的肝脾血流动力学、降低纤维化指标。
Objective ： To explore the effect of Zexie Decoction in the treatment of cirrhosis patients with portal hypertension, in terms of hemodynamics and liver fibrosis. Methods ： 100 cases of cirrhosis with portal hyper- tension were randomly divided into the treatment group and the control group, with 50 eases in each group. Both groups were given conventional treatment, based on which the treatment group was also given Zexie Decoction. Clinical efficacy, hemodynamic changes, and the serum levels of HA, PCⅢ, LN and IV - C were observed after three months. Results ： After three - month treatment, the diameters and the blood flows of he-patic portal vein and of splenic vein in the treatment group were all smaller than those of the control group with significant differences （ P 〈 0.05 ） ; the blood flow velocities of hepatic portal vein and of splenic vein were both faster than those of the control group with a significant difference （ P 〈 0.05 ） ; the levels of HA, PCIII, LN and Ⅳ- C were lower in the treatment group than those in the control group, and the differences were statistically significant （ P 〈 0.05 ） ; the total effective rate in the treatment group was 92% , which was higher than that 76% in the control group, and the difference was statistically significant （P 〈 0.05 ）. Conclusion ： Zexie Decoction can significantly improve hepatic hemodynamies and splenic hemodynamies, and it can reduce fibro- sis for cirrhosis patients with portal hypertension.
