摘要
目的:调查我院儿科住院患者超药品说明书用药情况,并进行分析评价,为儿科患者的规范合理用药提供参考。方法:采用回顾性调查,随机抽取2016年2月1日到3月31日住院患儿病例600例,逐一阅读药品包装所附药品说明书,判断超药品说明书用药处方并分析其原因。结果:共纳入住院病例600例,分析用药医嘱3 769条,涉及药品157种。按病例数、用药医嘱、药物品种计,超药品说明书用药分别为93.2%、31.8%和30.6%。各类型超药品说明书用药发生率分别为超适用人群(66.0%)、超适应证(25.6%)、超给药途径(22.6%)、超给药剂量(11.1%);超药品说明书用药医嘱占据第一位的药物为心血管系统药(26.5%),其次为呼吸系统类药物(21.6%)。结论:儿科住院患者超药品说明书用药现象普遍,相关管理部门应对超药品说明书用药行为进行规范,保障患儿用药安全,规避用药风险。
Objective:To research the actuality of off-label drug use on pediatric inpatients in a certain hospital, and to analyze and comment on these cases to provide a reference for rational drug use and standard medication on children clinically. Methods: With the method of retrospective investigation, 600 hospitalized cases of pediatrics from February 1,2016 to March 31,2016 were selected at ran- dom. To read the package inserts of drug one by one. Then the prescriptions which are off-label use were screened out and analyzed their reasons. Results: A total of 600 hospitalized children were included, and 3769 medical orders were analyzed, involving 157 typed. By the number of prescriptions,medication records and drug types,the incidence of off-label drug use were 93.2% , 31.8% and 30. 6%. The type of off-label drug use mainly included off-label suitable crowd(66. 0% ), off-label indication (25.6%) ,exceeding route of administra- tion (22. 6% ), off-label dosage (11.1% ). The top-one drugs in the list of the incidence of off-label drug use were drugs for cardiovascular drugs (26. 5% ), the second were drugs for respiratory system (21.6%). Conclusion: The phenomenon of off-label drug use on pediatric inpatient is pervasive in the hospital. Relevant administrative departments should formulate the management system of off-label drug use which is supposed to be standardized reasonably, in order to guarantee safe medications for children, and to avoid drug risks.
