磁共振成像联合血清糖类抗原与癌胚抗原诊断胰腺癌的价值 被引量:9

Value of magnetic resonance imaging(MRI) combined with carbohydrate antigen and carcino-embryonic antigen detection in the diagnosis of pancreatic carcinoma

摘要 目的探讨磁共振成像联合血清糖类抗原19-9(CA19-9)、糖抗原125(CA125)和癌胚抗原(CEA)在诊断胰腺癌中的价值。方法回顾性分析我院收治的55例胰腺癌患者的临床资料,所有患者均进行磁共振常规扫描、脂肪抑制、增强扫描。比较胰腺癌与正常胰腺组织磁共振扫描参数,并与血清检测CA19-9、CA125和CEA进行对比分析。结果胰腺癌组患者T1WI呈低或稍低信号(42/55),T2WI呈略高信号(42/55),脂肪抑制序列肿瘤呈低信号(45/55),符合胰腺癌典型特征。胰腺癌组患者的血清CA19-9、CA125和CEA水平明显高于正常对照组(P<0.05)。单项检测中,CA19-9的阳性率为78.18%,CA125的阳性率为66.45%,CEA的阳性率为51.91%,CA19-9阳性率最高,CEA的阳性率最低;联合检测的阳性率为90.91%。单纯磁共振成像诊断胰腺癌的诊断符合率为89.09%,联合血清CA19-9、CA125与CEA诊断胰腺癌的诊断符合率高达96.36%。结论磁共振成像联合CA19-9、CA125和CEA水平检测可提高胰腺癌的诊断符合率,具备较高的临床应用价值。 Objective To investigate the value of MRI combined with carbohydrate antigen 19-9 (CA19-9), carbohydrate antigen 125 (CA125) and carcino-embryonic antigen (CEA) in the diagnosis of pancreatic carcinoma. Methods The clinical data of 50 pancreatic carcinoma patients admitted in our hospital were retrospectively analyzed. All the patients received routine MRI scan, fat suppression and enhancement scanning. The MRI parameters of pancreatic carcinoma and normal pancreatic tissue were compared, and compared with serum CA19 -9, CA125 and CEA detection results. Results In pancreatic carcinoma group, the T1WI showed low or slightly low signal (42/55), and T2WI showed slightly higher signal (42/55), fat suppression carcinoma showed low signal (45/55), all these signals were in accordance with the typical characteristics of pancreatic carcinoma. The serum CA19-9, CA125 and CEA levels in the pancreatic carcinoma group were higher than those in the control group (P〈0.05). In single detection, the positive rate of CA19-9 was 78.18%, the positive rate of CA125 was 66.45%, the positive rate of CEA was 51.91%; the positive rate of CA19-9 was the highest, and the positive rate of CEA was the lowest. The positive rate of combination detection was 90.91%. In the diagnosis of pancreatic carcinoma, the diagnostic accordance rate of MRI was 89.09%, and the diagnostic accordance rate of MRI combined with CA19 -9, CA125 and CEA antigen detection was 96.36% . Conclusion MRI combined with CA19 -9, CA125 and CEA detection can improve the diagnostic accordance rate of pancreatic carcinoma, which has high clinical value.
