摘要
目的:探讨后路微创小切口减压联合经皮椎弓根螺钉复位内固定术治疗胸腰椎骨折的效果。方法:选取胸腰椎脊柱骨折患者150例,将患者随机分为观察组(n=71)和对照组(n=79),观察组给予后路微创小切口减压联合经皮椎弓根螺钉复位内固定术,对照组给予开放后路减压椎弓根螺钉复位内固定术,观察两组手术时间、术中出血量、椎体高度和后凸Cobb角等指标。结果:观察组手术时间明显长于对照组(P<0.05);观察组肌肉剥离长度、术中出血量和术后引流量明显少于对照组(P<0.05);观察组术后2周VAS和ODI明显低于对照组(P<0.05);观察组和对照组术后6个月椎体高度、后凸Cobb角均较术前明显改善(P<0.05),两组术后6个月椎体高度、后凸Cobb角比较差异无统计学意义(P>0.05)。结论:后路微创小切口减压联合经皮椎弓根螺钉复位内固定术治疗胸腰椎骨折,具有切口小、出血少、伤口疼痛轻等优点。
Objective: To investigate the effect of minimally invasive posterior decompression combined with percutaneous pedicle screw reduction and internal fixation in the treatment of thoracolumbar fractures.Methods:In our hospital for the treatment of thoracolumbar spine fractures in patients with 150 cases,the patients were randomly divided into observation group (n=71) and control group (n=79),the observation group treated with posterior decompression combined with minimally invasive percutaneous pedicle screw fixation,the control group was given open posterior decompression and pedicle screw fixation,the operation time two groups of operation,bleeding volume,vertebral height and kyphosis Cobb angle indicator.Results:The operation time of the observation group was significantly longer than the control group (0.05);observation group muscle stripping length,intraoperative bleeding and postoperative drainage were significantly less than the control group (P〈0.05);2 weeks of observation VAS and ODI group were significantly lower than the control group (P〈0.05);the observation group and the control group after 6 months of vertebral height,kyphosis Cobb angle were significantly improved compared with preoperative (P〈0.05),the two groups 6 months after surgery,no significant vertebral height,kyphosis Cobb angle difference (P〉0.05).Conclusion:Minimally invasive posterior decompression combined with percutaneous pedicle screw reduction and internal fixation for the treatment of thoracolumbar fractures,has small incision,less bleeding,less pain and other advantages,is worthy of clinical use.
