摘要
目的:探讨鼻内镜手术联合低温等离子刀辅助治疗对慢性鼻窦炎患者术后鼻腔容积及肺通气功能的影响。方法:选择2013年1月至2015年6月我院收治的慢性鼻窦炎患者60例作为本次研究对象,根据随机数字表法分成对照组和观察组,每组各30例,其中对照组采用传统性的鼻内镜手术联合下鼻甲骨折外移术治疗,观察组患者给予鼻内镜手术联合低温等离子刀辅助治疗,对比分析两组患者术后12个月治疗效果、术前和术后8周及12个月嗅觉功能和鼻腔容积、术前和术后12个月肺通气功能情况。结果:观察组和对照组患者临床治疗有效率分别为96.67%和80.00%,差异具有统计学意义(P<0.05);术后8周和12个月,两组患者嗅觉功能评分均显著降低,鼻腔容积均显著增加,差异均具有统计学意义(P<0.05);观察组患者术后8周和术后12月时的嗅觉功能和鼻腔容积改善均显著优于对照组,差异具有统计学意义(P<0.05);手术后,两组患者各项肺通气指标(FEV1、FVC、EV1/FVC和PEF)均明显改善,差异具有统计学意义(P<0.05);观察组患者术后12个月时各项肺功能通气指标改善程度均明显著优于对照组,差异具有统计学意义(P<0.05)。结论:鼻内镜手术联合低温等离子刀辅助治疗慢性鼻窦炎疗效确切,能够显著改善患者鼻腔容积及肺通气功能。
Objective: To explore the effect of nasal endoscopic surgery combined with low-temperature plasma cutter on nasal cavity volume and pulmonary ventilation function in patients with chronic nasal sinusitis after postoperation. Methods: 60 patients of chronic nasal sinusitis treated in our hospital from January 2014 to January2015 as research objects,and divided into control group and observation group( 30 cases in each group). Control group was treated with nasal endoscopic surgery combined with inferior turbinate out-fracture,and observation group was treated with nasal endoscopic surgery combined with low-temperature plasma cutter. The therapeutic effect at 12 months after operation,the olfactory function and nasal cavity volume before operation and 8 weeks and 12 months after operation,the pulmonary ventilation function before operation and 12 months after operation were compared between two groups. Results: The total effective rate of observation group was 96. 67%,was significantly higher than control group( 80. 00%),two sets of data between a statistically significant difference( P〈0. 05). At 8 weeks and 12 months after operation,the olfactory function scores of two groups were significantly reduced and the nasal cavity volume of two groups were noticeable increase,the difference of statistical meaning between two groups( P〈0. 05). At 8 weeks and 12 months after operation,the olfactory function and nasal cavity volume of observation group improvement were better than control group,the difference of statistical meaning between two groups( P〈0. 05). At 12 months after operation,the pulmonary ventilation index( FEV1、FVC、EV1/FVC and PEF) of two groups were improved significantly,the difference of statistical meaning between two groups( P〈0. 05). At 12 months after operation,the pulmonary ventilation index of observation group improvement were better than control group,the difference of statistical meaning between two groups( P〈0. 05). Conclusion: nasal endoscopic surgery combined with low-temperature plasma cutter have certain curative effect on the treatment of chronic nasal sinusitis,and can significantly improve the patient's nasal cavity volume and pulmonary ventilation function.
