摘要
目的探讨胆管切开取石术与肝段切除治疗肝内胆管结石的临床疗效。方法选择2010年12月至2014年12月该院收治的155例肝内胆管结石患者为研究对象,根据手术方式将其分为观察组和对照组,其中对照组接受胆管切开取石术治疗,而观察组则运用肝段切除术治疗,对两组的术后并发症、手术情况、复发率及治疗效果进行比较和分析。结果两组患者均顺利完成手术,观察组的术后住院时间、术中出血量及手术时间均优于对照组,差异均有统计学意义(P<0.05);观察组与对照组在治疗优良率方面比较,差异有统计学意义(P<0.05);与对照组比较,观察组的复发率及并发症总发生率均较低,差异有统计学意义(P<0.05)。结论临床上运用肝段切除术治疗肝内胆管结石,不仅具有复发率低、安全性高、创伤小等特点,还能获得较好的疗效,减轻患者的经济负担,改善预后,值得推广。
Objective To investigate the clinical effects of hepatic segment resection and cholangiolithotomy in treating intrahepatic duct clculi. Methods One hundred and fifty-five cases of intrahepatic duct calculi in our hospital from December2010 to December 2014 were selected as the research subjects and divided into the observation group and control group according to the operation mode. The control group received cholangiolithotomy,while the observation group used hepatic segment resection.The postoperative complications,operion situation,recurrence rate and treatment effect were analyzed and compared between the two groups. Results The two groups successfully completed surgery. The postoperative hospitalization time,intraoperative blood loss volume and operation time in the observation group were less than those in the control group,the differences between the two groups were statistically significant（P〈0.05）; the total effective rate had statistically significant difference between the two groups（P〈0.05）; compared with the control group,the recurrence rate and occurrence rate of complications in the observation group were lower than those in the control group,the differences between the two groups were statistically significant（P〈0.05）. Conclusion The clinical use of hepatic segment resection in the treatment of intrahepatic duct calculi not only has the advantages of low recurrence rate,high security and small trauma,but also can obtain better curative effect,reduces the patient′s economic burden,improves the prognosis,and is worth promoting.
