摘要
目的探讨帕金森病患者静息态功能磁共振(rsf MRI)的临床应用价值。方法2013年8月到2016年1月选择在我院诊治的帕金森病患者45例(帕金森病组)和和门诊体检健康者45例(对照组),都进行帕金森病统一评定量表(UPDRS)和蒙特利尔认知评估量表(MOCA)评定,同时进行rsf MRI的影像学观察与变量Peak T值计算和相关性分析。结果帕金森病的UPDRS评分明显高于对照组,而MOCA 6个维度评分都明显低于对照组,对比差异都有统计学意义(P<0.05)。与对照组相比,帕金森病组的右小脑后叶、距状裂、左额中回、左中央前回的脑区功能显著增加;经过计算,上述组织的rsf MRI Peak T值在两组间对比差异也有统计学意义(P<0.05)。在帕金森病组中,Spearman相关分析结果显示脑区功能平均Peak T值与示UPDRS评分呈现正相关(r=0.341,P<0.05),与视空间/执行功能评分呈负相关(r=-0.332,P<0.05)。结论帕金森病患者rsf MRI临床应用能进行早期诊断与病情分析,能有效反映患者的认知功能情况,有很好的应用价值。
Objective To investigate the clinical value of rsfMRI in patients with Parkinson＇s disease.Methods From August 2013 to January 2016, 45 cases of in-patients diagnosed as Parkinson＇s disease （PD group） and 45 cases （control group） of outpatient healthy people were selected, the Unified Parkinson＇s Disease Rating Scale （table （UPDRS） and Montreal cognitive assessment table （MoCA） were used to evaluation. At the same time, the image of rsflVIRl peak T values were given calculation and correlation analysis. Results The UPDRS scores of PD group were significantly higher than that of the control group, while the MOCA scores in 6 dimensions were significantly lower than those in the control group, and compared the difference were also statistically significant （P〈0.05）. Compared with the control group, th- right cerebellum posterior lobe, calcarine, left middle frontal gyrus, left precentral gyrus functional areas of the brain in the PD group were significantly increased. After calculation, the organization of rsflVIRl peak T values between the two groups in the difference were also statistical significance （P〈0.05）. In PD group, Spearman correlation analysis showed that the average T Peak value and UPDRS score were showed positive correlation （r=0.341 P〈0.05）, and were neganve correlated with the visual space/executive function score （r=-0.332. P〈0.05）. Conclusion The clinical application of rsfMRI in patients with PD can be used for early diagnosis and analysis, which can effectively reflect the cognitive function of patients, and has good application values.
