摘要
目的探讨帕洛诺司琼对胸腔镜术应用舒芬太尼后镇痛期恶心呕吐的防治效果。方法选择2016月2月至2017年2月在医院接受胸腔镜手术的患者108例,按随机数字表法分为对照组和观察组,各54例。两组患者均采用相同的麻醉方式,在此基础上,对照组患者于切皮前10 min静脉注射托烷司琼5 mg,观察组患者则于切皮前10 min静脉注射帕洛诺司琼2.5 mg。结果两组患者在手术即刻、术中30 min及60 min时的平均动脉压、心率比较,均无明显差异(P>0.05);观察组患者术后4,12 h恶心呕吐发生率与对照组相比,无明显差异(P>0.05),而术后24,48 h显著低于对照组(P<0.05)。结论帕洛诺司琼防治胸腔镜术应用舒芬太尼后镇痛期恶心呕吐效果明显,且安全性较高,尤其对延迟性呕吐的防治更有优势,值得临床推广。
Objective To investigate the effect of palonosetron in preventing nausea and vomiting during analgesia period with sufentanil after video-assisted thoracoscopic surgery( VATS). Methods Totally 108 patients underwent VATS from February 2016 to February2017 in our hospital were selected and divided into the control group and the observation group by random number table method,54 cases in each group. The two groups were treated by the same anesthesia method,on this basis,the control group was injected intravenously with 5 mg tropisetron 10 min before skin incision,while the observation group was injected intravenously with 2. 5 mg palonosetron 10 min before skin incision. Results At the time of the operation,30 min and 60 min during the operation,the mean arterial pressure and heart rate in the two groups showed no obvious difference( P 〈 0. 05). 4,12 h after operation,the incidence rate of nausea and vomiting had no significant difference between the two groups( P 〈 0. 05),but 24,48 h after operation,the incidence rate of nausea and vomiting in the observation group was significantly lower than that in the control group( P 〈 0. 05). Conclusion Palonosetron for preventing nausea and vomiting during analgesia period with sufentanil after VATS has good effect and high safety,especially has more advantages for prevention and control for delayed vomiting,it is worthy of clinical promotion.
