摘要
目的:分析对比用带骨髌韧带重建法与直接钻孔缝合法重建指伸肌腱止点断裂的临床疗效。方法:选取2012年5月至2016年3月东莞市清溪医院收治的手指伸肌腱止点断裂(锤状指)的110例患者,随机分为A、B两组,每组55例。A组采用带骨髌韧带重建法,B组采用直接钻孔缝合法。观察两组患者术后的临床疗效。结果:全部患者术后3~8个月随访,A组伸肌腱止点再次断裂2例,远侧指间关节僵硬致屈、伸活动受限5例,伤指远指间关节主动屈伸活动恢复良好、无疼痛48例;B组伸肌腱止点再次断裂8例,远侧指间关节僵硬致屈、伸活动受限10例,伤指远指间关节主动屈伸活动恢复良好、无疼痛37例。治疗后两组疗效比较,差异具有统计学意义(P<0.05)。结论:用带骨髌韧带重建指伸肌腱止点断裂综合疗效优于直接钻孔缝合法。
Objective To compare and analyze the clinical effect of bone patellar ligament reconstruction method and direct drilling suture method in the reconstruction of finger extensor tendon check point rupture. Methods From May 2012 to March 2016, 110 cases of patients with finger extensor tendon check point rupture (hammer finger) treated in our hospital were selected and randomly divided into A group and B group, each group with 55 cases. A group was operated by bone patellar ligament reconstruction method, and B group was operated by directed drilling suture method. Observe and compare the clinical effects of two groups. Results All patients received follow-up visit after 3-8 months of the operation, in the group A, extensor tendon check point ruptured again in 2 cases, 5 cases of finger flexion activity limitation caused by the distal interphalangeal joint stiffness,48 cases had good effects of active flexion and extension for the distal interphalangeal joint, no pain; and in the group B, extensor tendon check point ruptured again in 8 cases, 10 cases of finger flexion activity limitation caused by the distal interphalangeal joint stiffness, 37cases had good effects of active flexion and extension for the distal interphalangeal joint, no pain. There was a statistically significant difference between the two groups, P 〈 0.05. Conclusion The comprehensive curative effect of the bone patellar ligament reconstruction method is better than that of the direct drilling suture method in the reconstruction of finger extensor tendon check point rupture.
