摘要
目的探讨儿童先天性上睑下垂矫正术后眼表的变化。方法收集儿童先天性上睑下垂患儿28人33眼,分别用提上睑肌缩短术或额肌肌瓣悬吊术两种术式矫正,术前及术后第2 d、7 d、14 d、1个月和3个月,检测泪膜破裂时间(BUT)、泪液分泌试验(St)、角膜荧光素染色(FL)的变化,对结果进行统计学分析;观察睑裂闭合不全情况、Bell氏现象、上睑睫毛方向的变化情况。结果所有患儿术后第2 d、7d,角膜荧光素染色上皮有着点,FL平均积分均有所增加,BUT明显缩短,与术前相比有统计学差异(P<0.05),14d、1个月和3个月FL及BUT值基本恢复,与术前相比无统计学差异(P>0.05);St泪液分泌术后第2d、7d、14d、1个月和3个月数值与术前相比无统计学差异(P>0.05);2种术式相比,BUT、St、FL指标差异无统计学意义(P>0.05)。结论儿童先天性上睑下垂矫正术后早期角膜上皮有一定程度的损伤及泪膜稳定性破坏,泪液分泌量无明显影响,且发生与睑裂闭合不全、倒睫、瞬目减少、Bell氏现象消失有关。手术方式对眼表的改变无明显差异。
Objective To investigate the changes of tear film in patients with children congenital ptosis after surgical correc- tion. Methods Totally 33 eyes （28 patients） with children congenital ptosis were treated with two suspension of muscle. Bell phenome- non, angle eyelash, winking frequency, eyelid closure, fluorescent staining of the comeal and tear film break - up time （ BUT ） and Schirmer test were observed pre - and post - operation and on the 2nd and 7th day and in the first and 3rd month after operation. All the results were analyzed with statistical soft. Results There was significantly difference in the comeal epithelial staining spots and tear film break- up time before and after surgery with one day and seven day. There was no significant difference in the Schirmer test （P 〉 O. 05 ）. The changes of the ocular surfaces had no significant difference between the patients with eyelid elevator excision or frontalis flap suspension （P 〉 0.05）. Conclusion The cornea/epithelial and the tear film stability are slightly damaged in the early period after sur- gical correction for ehildren congenital ptosis, hut the Schirmer test do not change. The occurrence of corneal injury is mainly related with Bell phenomenon and angle eyelash and winking frequency and eyelid closure. The surgical methods for correcting congenital ptosis do not affect the changes of ocular surface.
