摘要
目的探讨门急诊退药原因及预防措施。方法选择2015年1月—2016年12月江苏省南通大学附属医院分院门急诊所有退药处方作为研究对象,包括内科、妇产科、外科、五官科、儿科、中西医结合科、康复科、针灸推拿科等临床科室,将门急诊退药记录单据集合到一起,按照退药原因分析。结果近2年该院门急诊调配处方中退药处方140张;原因包括药品因素(药物不良反应、药品质量、近效期)68张(47.14%,68/140)、医生因素(处方录错、用药不当、重复开药、开药过多等)40张(28.57%,40/140)、患者因素(已有所需药品、对药不认可、费用问题、病情变化、取消检查或手术项目等)32张(22.86%,32/140)。结论门急诊退药原因复杂,涉及药品因素、医生因素及患者因素,加强医院管理完善退药制度进行,提高医师用药水平及职业道德感,增加药物的咨询窗口正确解答患者的疑问,提高药师审核处方能力等以减少退药现象的发生。
Objective To explore the causes and preventive measures of drug withdrawal causes in outpatient and emergency department. Methods All withdrawal prescriptions in outpatient and emergency department in our hospital from January2015 to December 2016 were selected as the research objects including the department of internal medicine, department of gynecology and obstetrics, department of surgery, department of ophthalmology and otorhinolaryngology, department of pediatrics, department of integrated traditional and western medicine, department of rehabilitation and acupuncture and massage department, and the records were analyzed according to the withdrawal causes. Results In recent two years, there were 140 pieces of withdrawal prescriptions in the outpatient and emergency department, and the causes include the drug factor（drug adverse reactions, drug quality, short-term period） 68 pieces（47.14%,68/140）, doctor facto（prescription records errors, improper medication, repeated prescription and redundant prescription） 40 pieces（28.57%,40/140）, and the patient factor（required medicine available, disapproval of drugs, cost problem, disease changes and canceling examination or operation items） 32 pieces（22.86%,32/140）. Conclusion The drug withdraw causes in the outpatient and emergency department is complex, involving drug factor, doctor factor and patient factor, and we should enhance the improvement of drug withdraw system, improve the medication level and occupational morality of physicians, increase the drug consultation window to accurately reply the patients＇ questionnaires and improve the prescription audit ability of pharmacists in order to reduce the occurrence of phenomenon of drug withdrawal.
