摘要
目的观察运动康复结合口服氨基葡萄糖治疗髌骨软化症的临床疗效。方法回顾性分析天津医院于2015-03—2016-09间诊治的髌骨软化症患者80例。将患者随机分为两组,对照组40例采用单纯运动康复治疗,包括靠墙静蹲、抗阻伸膝、髌骨按摩、股四头肌牵伸治疗;治疗组40例采用运动康复结合口服氨基葡萄糖治疗。采用疼痛数字评分法(NRS)对患者治疗前和治疗后进行疼痛评分。参照lysholm膝关节评分标准对患者治疗前和治疗后进行功能评分,依据尼莫地平评分法计算疗效指数。疗效优和良为有效,疗效可和差为无效,比较两组患者在疼痛评分和疗效方面的差异。结果经过治疗,两组患者膝关节疼痛较治疗前均有明显减轻,差异有统计学意义(P<0.05)。与对照组比较,治疗组改善更为明显(P<0.05)。治疗组疗效:优29例、良8例、可2例、差1例;对照组疗效:优17例、良12例、可6例、差5例。治疗组和对照组总有效率分别为92.5%和72.5%,两组总有效率比较差异有统计学意义(P<0.05)。结论运动康复结合口服氨基葡萄糖综合治疗髌骨软化症疗效肯定,值得临床推广。
Objective To investigate the clinical effects of chondromalacia patellae for the treatment of exercise rehabilitation combined with glucosamine. Methods The retrospective analysis included 80 patients with chondromalacia patellae treated in the hospital from March,2015 to September,2016. With simple random method,the patients were divided into two groups. The control group included 40 patients taking the treatment of exercise rehabilitation: squatting down against the wall,extending the knee with resistance,massaging the patellar and stretching the quadriceps femoris. The treatment group included 40 patients taking the treatment of exercise rehabilitation combined with glucosamine. Through numerical rating scale( NRS),all patients were measured before treatment and after treatment. Through lysholm knee scoring criteria,all patients were measured before treatment and after treatment.Curative effect index were calculated on the basis of nimodipine scoring method. Excellent and good belonged to effective,and fair and poor belonged to ineffective. The differences of NRS and curative effect were compared in two groups. Results After treatment,the knee joint aches of two groups are significantly decreased than before treatment( P〈0. 05). Treatment group improves more significantly as compared with control group( P〈0. 05). In curative effect of treatment group: there are 29 excellent patients,8 good patients,2 fair patients and 1 poor patient; in curative effect of control group: excellent in 17 patients,good in 12 patients,fair in 6patients,poor in 5 patients. The total effective rate is 92. 5% in treatment group and 72. 5% in control group. The difference of total effective rate between the two groups is statistically significant( P〈0. 05). Conclusion Exercise rehabilitation combined glucosamine can be the effective treatment for the chondromalacia patellae patients,so it is worth clinical promotion.
